The maximum annual funding includes a 30% contingency amount. No portion of the contingency may be billed for without prior written approval from Alberta Health Services for use of the contingency. AHS reserves the right to extend the contract term or volumes paid out at the rates specified in Schedule “B”, Section II, “Service Fees.” Commencing April 1, 2014, the Service Fees may be adjusted, no more than annually, for inflation. The adjustment shall be based on the mid-point between the average of the applicable public health care sector wage settlement for Calgary, Alberta and the Consumer Price Index (CPI) indicator “Alberta All-Items excluding Energy, excluding Tobacco and excluding Alcohol” for the previous Fiscal year and such adjustment shall follow the guiding principles defined by the Statistics Canada methodology. Any adjustment in Service Fees shall not exceed the funding granted to AHS to cover the cost of the operations of AHS in respect of the Services for the applicable Fiscal Year.   • Total Contract Amount The contract total for services shall not exceed $1,200,000. Pricing shall be per Exhibit F attached.

  • MINIMUM MONTHLY PAYMENT We will mail you a statement every month if your account has a balance. You agree that you will pay each month not less than the minimum monthly payment by the payment due date. The minimum monthly payment will be 3.0% for Classic accounts or 2% for Platinum accounts of your outstanding balance (“New Balance”) or $25.00, whichever is greater. If your outstanding balance is $25.00 or less, you agree to pay the balance in full. You may pay in full for all your purchases and cash advances each month, or you may repay in monthly installments. We can accept late payments or partial payments, or checks, drafts, or money orders marked “payment in full” without prejudice to our rights under this Agreement, which are hereby explicitly reserved.   • Maximum Amount Payable The maximum amount payable under this contract without modification is shown in Attachment E, Fee Schedule. Payment under this contract beyond the end of the current fiscal biennium is subject to availability of appropriated funds. If funds are not appropriated, this contract shall be terminated immediately with no liability to either party.

  • Average Contribution Amount For purposes of this Agreement, to ensure that all employees enrolled in health insurance through the City’s HSS are making premium contributions under the Percentage-Based Contribution Model, and therefore have a stake in controlling the long term growth in health insurance costs, it is agreed that, to the extent the City's health insurance premium contribution under the Percentage-Based Contribution Model is less than the “average contribution,” as established under Charter section A8.428(b), then, in addition to the City’s contribution, payments toward the balance of the health insurance premium under the Percentage-Based Contribution Model shall be deemed to apply to the annual “average contribution.” The parties intend that the City’s contribution toward employee health insurance premiums will not exceed the amount established under the Percentage-Based Contribution Model.
