Major Organ /Bone Marrow Transplant i. Consultation Paper stating the presenting complaints with duration, past medical history with duration, treatment and medication advised.
Major Organ /Bone Marrow Transplant the actual undergoing of a transplant of: • One of the following human organs: heart, lung, liver, kidney, pancreas, that resulted from irreversible end-stage failure of the relevant organ, or • Human bone marrow using haematopoietic stem cells. The undergoing of a transplant has to be confirmed by a specialist medical practitioner. The following are excluded: • Other stem-cell transplants • Where only Islets of Langerhans are transplanted.

Major Organ /Bone Marrow Transplant. The receipt of a transplant of: - Human bone marrow using hematopoietic stem cells preceded by total bone marrow ablation; or - One of the following human organs: heart, lung, liver, kidney, pancreas that resulted from irreversible end stage failure of the relevant organ. Other stem cell transplants are not covered.
Major Organ /Bone Marrow Transplant. The actual undergoing of a transplant of any of the below organ as a recipient or the inclusion on an official transplant waiting list for any of the below organ: - One of the following whole human organs: heart, lung, liver, kidney, pancreas or - Human bone marrow using haematopoietic stem cells preceded by total bone marrow ablation. The transplant must be medically necessary and based on objective confirmation of organ failure. Other than the above stem cell transplants are excluded.
