Maintaining Individual Medical Record. 37 Provider shall maintain a medical record for each individual to whom Provider 38 renders behavioral healthcare services. Provider shall establish each individual’s 39 medical record upon the individual’s first encounter with Provider. The individual’s 40 medical record shall contain all information required by state and federal law, 41 generally accepted and prevailing professional practice, applicable government 42 sponsored health programs, and all North Sound BH-ASO P&Ps. Provider shall 43 retain all such records for at least 10 years.
