Common use of Limited Data Set Clause in Contracts

Limited Data Set. Covered Entity agrees that the Protected Health Information as identified in Attachment A to this Agreement (the “Limited Data Set”) will be disclosed by either the Covered Entity and/or by Administrator acting on behalf of the Covered Entity to Data Recipient. For the avoidance of doubt, the Limited Data Set shall not include any of the following identifiers of individuals or of relatives, employers, or household members of individuals: names; postal address information other than town, cities, states, and zip codes; telephone or fax numbers; email addresses, URLs, or IP addresses; social security numbers; medical record numbers; health plan beneficiary numbers; account numbers; certificate or license numbers; vehicle identification or serial numbers (including license plate numbers); device identifiers or serial numbers; biometric identifiers (such as voice and fingerprints); and full face photographs or comparable images.
