Life, Accidental Death and Dismemberment and Short Term and Long Term Disability Benefits. The Company will pay the full cost of the Basic Life Insurance, Accidental Death and Dismemberment, Short-term Disability and Basic Long Term Disability Plans for eligible employees. The Company will provide access to an employee-paid Supplemental Life Plan, Supplemental Accidental Death and Dismemberment Plan, a ten percent supplemental Long Term Disability Plan benefit, and Health Care and Dependent Care Spending Account Plans under the same terms, conditions and limitations as described in the Summary Plan Description for BCA Puget Sound nonunion salaried employees in effect as of January 1, 2016. Company and employee cost will be based on SPEEA claims experience. The Company will pay the full cost of the Life Insurance, Accidental Death and Dismemberment, and Short Term Disability Plans for eligible employees.

Life, Accidental Death and Dismemberment. (AD&D) INSURANCE AND VOLUNTARY BENEFITS All CFA-represented employees shall receive city paid life insurance in an amount equal to their base salary up to a maximum base salary of $200,000. To determine the benefit, the amount of insurance is rounded to the next higher $1,000 multiple, unless the amount equals a $1,000 multiple. Effective January 1, 2020, all CFA-represented employees shall receive city paid Accidental Death and Dismemberment (AD&D) insurance in an amount equal to their base salary up to a maximum base salary of $200,000. To determine the benefit, the amount of insurance is rounded to the next higher $1,000 multiple, unless the amount equals a $1,000 multiple. The city provides various voluntary benefits available at the employee’s cost. Employees may select among various levels of coverage. Life, Accidental Death and Dismemberment. INSURANCE Employees on active payroll, defined as those actively working, who have completed their probationary period with the Company shall be eligible for the life insurance benefit of one and one half (1.5) times annual earnings to a maximum of $60,000 and basic accident death and dismemberment insurance with benefits equal to the amount paid under the group life insurance plan. Employees with more than three (3) but less than twelve (12) months active service, will pay fifty percent (50%) of the monthly premiums by payroll deductions. The Company will pay the total insurance premiums for employees with more than twelve (12) months service. A list will be put together identifying the Province of Ontario certified licensed massage therapists in the area for employees.

  • Accidental Death and Dismemberment The Employer agrees to provide all active full-time employees with Accidental Death and Dismemberment benefit coverage equal to one (1) times their annual earnings in case of accidental death. Coverage is also provided for other losses such as speech and hearing, use of arms and legs, etc.

  • Accidental Death and Dismemberment Coverage An employee may purchase accidental death and dismemberment coverage that provides principal sum benefits in amounts ranging from five thousand dollars ($5,000) to one hundred thousand dollars ($100,000). Payment is made only for accidental bodily injury or death and may vary, depending upon the extent of dismemberment. An employee may also purchase from five thousand dollars ($5,000) to twenty-five thousand dollars ($25,000) in coverage for his/her spouse, but not in excess of the amount carried by the employee.

  • Group Life and Accidental Death and Dismemberment (a) The Employer will pay 100% of the premiums for the group life and accidental death and dismemberment insurance plans.

  • Accidental Death and Dismemberment Insurance The plan provides accidental death and dismemberment insurance coverage in an amount equal to your basic group life insurance (two times your current annual salary). Coverage is provided 24 hours per day, anywhere in the world, for any accident resulting in death, dismemberment, paralysis, loss of use, or loss of speech or hearing. If you sustain an injury caused by an accident occurring while the policy is in force which results in one of the following losses, within 365 days of the accident, the benefit shown will be paid to you.   • Life Insurance and Accidental Death and Dismemberment The Board shall provide without cost to the employee’s life insurance and accidental death and dismemberment insurance in the amount of $35,000.

  • Accidental Death & Dismemberment The Employer agrees to continue to make payroll deductions equivalent to the premiums for the current Accidental Death and Dismemberment Insurance Plan administered by the Union. All monies so deducted shall be remitted to the Union within fifteen (15) days of the end of the month in which the deductions were made along with a list of names of employees from whom the deductions were made.

  • Accidental Death Full twenty-four (24) hour Accidental Death coverage equivalent to coverage under the Group Life Plan.

  • Optional Life and Disability Coverages In order for coverage to become effective, the employee must be in active payroll status and not using sick leave on the first day following approval by the insurance company. If it is an open enrollment period, coverage may be applied for but will not become effective until the first day of the employee's return to work.

  • DEATH AND DISABILITY (a) The Employment Term shall terminate on the date of Employee's death, in which event Employee's Salary, reimbursable expenses and benefits owing to Employee through the date of Employee's death shall be paid to his estate. Employee's estate will not be entitled to any other compensation upon termination of this Agreement pursuant to this subparagraph 8(a).
