License and Certification. 24.1 Conditions of Employment When a license and/or certification is required as a part of the qualifications for a position prior to the appointment of an employee into the affected position, the employee will be responsible for the initial cost of the license and/or certification. Thereafter, the employee will be responsible for maintaining the license and/or certification and for all renewal costs.

License and Certification. Contractor shall, at all times during the term of this Contract, be licensed and certified for urinalysis per statutes contained in the California Health and Safety Code (Div. 5, Part 3, Article 3, Section 11881 and Article 4, Sections 11885 through 11896), and regulations contained in Sections 1160 through 1196 of the California Code of Regulations (Title 17, Group 5.5, Articles 1 through 8), Laboratory Improvement Amendment (CLIA), and College American Pathologist (CAP).
License and Certification. All present full-time employees and all future full-time employees covered by this Agreement shall be required as a condition of employment to maintain all active licenses and certifications that are required by the Employer, no later than twelve (12) months after the effective date of this Agreement. Any full-time employee who does not acquire the required license and certification within the above time period, and continue to maintain said licenses and certifications thereafter may be subject to corrective disciplinary action by the employer up to and including discharge. Any employee disciplined for failure to maintain the proper license and certification required by the Employer may grieve said discipline. Employees participating in job related course, the employer will pay cost for tuition, books, etc., for such approved courses. The employer will schedule the attendance of these training programs so all employees will have equal opportunity to attend.

License and Certification. The Provider shall meet state and federal service standards and applicable state licensure and certification requirements as expressed by state and federal rules and regulations applicable to the services covered by this contract. The Provider shall attach copies of its license or certification document and the most recent licensing or certification report concerning the Provider to this contract when returning the signed contract to the Purchaser. During the contract period, the Provider shall also send the Purchaser copies of any licensing inspection reports within 5 business days of receipt of such reports.
