Level of Coverage. (a) The person may elect to cover the same persons who were covered under the political subdivision's group health insurance plan or group health coverage plan through the person at the time the person left employment with the political subdivision, or the person may elect to discontinue coverage for one or more persons. A person who was not covered under the plan at the time the person to whom this chapter applies left employment with the political subdivision is not eligible for coverage under this chapter.
Level of Coverage. Effective July 1, 2022 or after approval by Voya Insurance, whichever is later, the City agrees to provide at its cost the basic life insurance benefit equal to two (2) times of the employee’s annual salary up to the maximum of $300,000 for all regular full-time employees and job share or regular part-time employees in permanent budgeted position covered by this MOU.

  • Agreement of Coverage  The Eligible Person and/or Dependent loses eligibility under Medicaid or Children's Health Insurance Program (CHIP). Coverage will begin only if SHL receives the completed enrollment form and any required Premium within 60 days of the date coverage ended.  Any other event which affects a Dependent’s eligibility. If the Subscriber fails to give notice which would have resulted in termination of coverage, SHL shall have the right to terminate coverage. A Dependent’s coverage terminates on the same day as the Subscriber.
