Legal Significance. Patient acknowledges that this Agreement is a legal document and creates certain rights and responsibilities. Patient also acknowledges having had a reasonable time to read and ask questions regarding this Agreement and to consider all alternatives and implications to signing this Agreement and does so without general or medical duress, and/or incapacitation. Thus, Patient is satisfied with the terms and conditions of the Agreement.
