Leave Request Procedure. 14.11 A written application for a leave of absence without pay or an extension of a leave of absence without pay shall be submitted to the appropriate administrator. The appropriate administrator shall determine if such a leave shall be granted and the conditions of such a leave.

Leave Request Procedure. 16 A. Employees are expected to be familiar with and are required to follow the 17 leave procedures as outlined in the Leave Request Procedures Section. 18 Leave requests for less than one full pay period should be handled with a 19 Personnel Leave Request Form attached to the time sheet.
