LATE APPLICANTS. If you do not apply for Family Coverage or to add dependents within the allotted time, you will have to wait until your Group's annual open enrollment period to do so. Your Family Coverage or the coverage for your additional dependents will then be effective on the first day of the month following the open enrollment peri­ od. Benefits will not be provided for any treatment of an illness or injury to a newborn child unless you have Family Coverage. (Remember, you must add the newborn child within 31 days of the date of birth.) CHANGING FROM FAMILY TO INDIVIDUAL COVERAGE You can apply to change from Family to Individual Coverage at any time. Your Group Administrator will give you the application and tell you the date that the change will be effective. Premiums will be adjusted accordingly.

LATE APPLICANTS. At the time you are hired, you are given an opportunity to elect certain benefits. If you waive participation in any of those programs for either yourself or your eligible dependents, you will only be allowed to apply for entry into the various Plans during Open Enrollment. You may be subject to benefits limitations on those plans for late entrants. Open Enrollment The open enrollment period allows employees to add or change their benefits coverage. Applications for Medical, Voluntary Dental, Voluntary Vision, Voluntary STD, and Voluntary Life Insurance may be submitted during this period. Once you have made a change, you cannot change that selection until the next Open Enrollment period (except in the case of a major life status change; see Special Enrollment). The open enrollment period is usually held during the month of December. Special Enrollment A Qualifying Event, such as certain life status changes--marriage, birth or adoption of a child or involuntary loss of medical and/or dental coverage, etc.--allow entry into these Plans as long as application for coverage is made within 30 days of the Qualifying Event. For specific details regarding Special Enrollment please refer your Summary Plan Description. Continuation of Coverage Federal law requires employers of 20 or more employees to give employees the right to choose to continue group health benefits provided by their group health plan for limited periods of time under certain circumstances, such as voluntary or involuntary job loss, reduction in the hours worked, transition between jobs, death, divorce, and other life events. Employees ordinarily may continue their health coverage for 18 months when their employment is terminated.

LATE APPLICANTS. You will be considered a Late Applicant if: • you apply for health coverage more than 60 days after losing group coverage, • you are transferring from an individual insurance plan, • you were not previously insured under a group insurance plan, or • you have enrolled under RTIP, even though current coverage has not ended, in order to coordinate benefits with another plan. Late Applicants are eligible for coverage under the RTIP Gold 750 plan as of the current or future date approved on the application, without Proof of Good Health. Coverage cannot be increased for 12 months, following the next January 1 renewal. C H A N G E S I N C O V E R A G E To ensure that coverage is kept up to date for yourself and your Dependant(s), it is important that you report any changes to OTIP.   • Insurance Application An employee on unpaid leave is eligible to continue to participate in group insurance programs if permitted under the insurance policy provisions. The employee shall pay the entire premium for such insurance commencing with the beginning of the leave and shall pay to the School District the monthly premium in advance, except as otherwise provided in law.
