Itemized Fees. Ongoing Primary Care is included with the Periodic Fee as described in Appendix 1. Please see a list of some of the chronic conditions we routinely treat (subject to change) on the Practice website (xxx.XxxxxxxxxxxXxxxxxxx.xxx). In-Office Procedures (subject to change) will be listed on the Practice website (xxx.XxxxxxxxxxxXxxxxxxx.xxx) and are available at no additional cost to members. Additional fees may apply for non-members. Laboratory Studies will be drawn in the offi ce and charged a discounted rate the lab provides to the practice. Additional fees may apply for non-members. Medications will be ordered in the most cost-effective manner possible for the Patient. If we dispense medications in the office these medications will be made available at cost without any significant markup in price. Pathology studies (typically skin biopsies) will be ordered in the most economical manner possible. Anticipated prices for these studies (subject to change) are listed on the Practice website (xxx.XxxxxxxxxxxXxxxxxxx.xxx). The cost of pathology studies will be charged to the patient a discounted rate as charged to the practice. Radiology studies will be ordered in the most cost-effective manner possible for the Patient. Surgery and specialist consults will be ordered in the most cost-effective manner possible for the Patient. As part of membership, patients have access to over 120 specialists through an electronic consult service known as RubiconMD. The practice will utilize this service for members as needed in order to provide comprehensive care when a specialists expertise is needed. This service may save patients signifi cant amounts of money and time in avoiding unnecessary referrals, however utilization of this service does not substitute a specialist referral when needed. Vaccinations are offered on a limited basis in our offi ce at this time due to the cost prohibitive nature of stocking a signifi cant supply. When necessary, we will try to help you obtain needed vaccinations elsewhere in the most cost-effective manner possible. Hospital Services are NOT covered by our membership plan. There are no itemized fees for office visits unless the Patient has more than twenty (20) scheduled in-office visits with Physician in a calendar year as explained in Appendix 3. Annual Wellness Visit with a wellness lab panel is provided at no additional cost once every 12 months. For existing patients, we will use the date of your last annual wellness visit with us to identify when you are due for your next annual visit. For those who have never received an annual wellness visit from us we will work with you to best schedule the visit. Please understand that the annual wellness visit should be scheduled in advance and there are no guarantees for immediate scheduling of this type of visit. In-Office Procedures that we are generally comfortable performing are listed on the Practice website. These are typically available at no additional cost unless otherwise designated, and these are also subject to change. Injections that we perform are listed on the Practice website. There is no cost for the injection procedure itself. Injectable medications may incur an additional charge.   • Expenses and Fees Except as expressly provided otherwise herein, all costs and expenses incurred in connection with this Agreement and the transactions contemplated hereby shall be paid by the Party incurring such expenses.

  • ATM Fees If you use an ATM to obtain a cash advance and the ATM is not operated by us, you may be charged an ATM surcharge by the ATM operator or an ATM network utilized for such a transaction. The ATM surcharge may be charged to your account if you complete the transaction.

  • Requesting Reimbursement Requests for reimbursements must be submitted via the ASAP system. Requests for reimbursement should coincide with normal billing patterns. Each request must be limited to the amount of disbursements made for the Federal share of direct project costs and the proportionate share of allowable indirect costs incurred during that billing period.

  • SERVICE CHARGES AND FEES You will pay fees incurred on the Account, including, without limitation, fees imposed by a Third Party through Your initiation of a Transaction. You will pay the service charges that We establish from time to time for The Services, including, without limitation, service charges for providing records regarding You that We are legally required to provide. You acknowledge receipt of a schedule of Our charges for The Services in effect at the time of acceptance of this Agreement. We may, from time to time, increase or decrease the service charges for The Services and provide notice of such changes by sending a notice to Your Contact Info, by posting notice at Our premises or on the Financial Institution’s website, by personal delivery, or by any other means We, acting reasonably, consider appropriate to bring the change to Your attention. Current service charges for The Services may be obtained by contacting Us or through the Financial Institution’s website. You are responsible for determining the then-current service charges for The Services You request, in advance of using The Services. By requesting The Services, You acknowledge Your agreement to pay service charges for The Services as requested as then in effect. The Financial Institution can deduct such obligations from Your Account (or other Accounts belonging to You with Us) when The Services are requested or performed. New or amended service charges and fees will become effective on the stated effective date following publication, when The Services are requested or performed, or when incurred, and in any event, no later than 30 days after publication by Us.

  • Program Fees Stripe will provide the Issuing Platform Services to you and the Stripe Issuing Services to Stripe Issuing Accountholders at the rates and for the fees described on the Stripe Pricing Page, unless you and Stripe otherwise agree in writing. In addition to the fees, you are also responsible for any penalties imposed on you or Stripe in relation to your use of the Issuing Platform Services. Fees and penalties may include periodic fees, foreign transaction fees, penalties for misuse, funds transfer fees, account maintenance fees, Card issuance or replacement fees, and penalties for late or failed payments.   • Monthly Fees ACS will xxxx Customer each month during the term of this Agreement based on number of "Actions" which occurred during the prior month. The definition of "Actions" and fees for each Action will be documented in each Task Order.   • Maintenance Fees All maintenance and similar fees in respect of any Purchased Assets that are due and payable prior to the Closing have been paid in full.
