INTENT TO ENROLL. I understand that I must be enrolled and making satisfactory academic progress to participate in student employment opportunities at NUNM. I understand that failure to maintain enrollment and satisfactory academic progress will result in suspension of employment privileges until I am in compliance. I understand that by using my FWS or STEP award (if any) during a period of non-enrollment, I am agreeing to enroll and make satisfactory academic progress during all terms for which I am receiving Financial Aid. I understand that failure to comply with enrollment and satisfactory academic progress for any/all terms for which I am awarded will result in my financial aid being reduced, suspended or canceled until I am in compliance. Initials I understand that failure to comply with employment policies and procedures will result in suspension and/or termination of my participation in the workstudy program. I acknowledge that I have received, read, and understand the Policies and Procedures Manual.

INTENT TO ENROLL. At least six months prior to matriculation at EU, the student confirms the intent to enroll by completing the admissions application. All students will follow established guidelines stated in the current EU college catalog for general admission to EU. EU will acknowledge the student’s intent to enroll and the student will submit an advance tuition deposit that will be credited towards the first semester fees.

  • Notice of Enrollment Said meeting and conferring shall not be subject to the impasse procedures in Government Code Section 3557. The Department sponsoring the NEO shall provide the foregoing information no less than five (5) business days prior to the NEO taking place. The Department will make best efforts to notify the Union NEO Coordinator of any last-minute changes. Onboarding of individual employees for administrative purposes is excluded from this notice requirement.

  • Voluntary Request for Assistance A member may voluntarily enter rehabilitation without a requirement of prior testing. A member who desires Employee Assistance Program (EAP) assistance may notify the City's EAP Administrator. A member who seeks voluntary assistance through his/her own service provider without notifying the City's EAP Administrator will not receive the protections from discipline afforded by this Section 17.11. Any member who does voluntarily seek assistance and who notifies the City's EAP Administrator before the member is asked to submit to a drug or alcohol test or is under investigation for drug or alcohol abuse, shall not be disciplined, but the member must:

  • Re-enrollment Any eligible employees who wish to join the Sick Leave Bank after their first year of eligibility will contribute two (2) days upon joining. Such membership may only be made during the month of October using the appropriate forms. The two (2) required days of leave shall be donated from their account upon enrollment in the Classified Employee Council (CEC).

  • Disenrollment Adverse Benefit Determination taken by the Division, or its Agent, to remove a Member's name from the monthly Member Listing report following the Division's receipt and approval of a request for Disenrollment or a determination that the Member is no longer eligible for Enrollment in the Contractor.

  • Voluntary Requests for Assistance The Employer shall take no adverse action against an employee who voluntarily seeks treatment or counseling. The Employer shall assist an employee seeking assistance by making available means by which referrals or treatment may be obtained (Employee Assistance Program). Such assistance shall be obtained at the employee's expense.

  • Request for Hearing The employee must file a written request for hearing within fifteen calendar days of receipt of the Notice of Dismissal or Suspension. Filing means receipt in the office designated no later than regular close of business on the last day of the filing period. Failure to file such request in a timely manner shall be deemed a waiver of the right to a hearing and the proposed action shall be effective upon action by the Governing Board without notice or hearing except as may be required in a board meeting agenda.

  • Notification of Acceptance of General Offer of Privacy Terms Upon execution of Exhibit “E”, General Offer of Privacy Terms, Subscribing LEA shall provide notice of such acceptance in writing and given by personal delivery, or e-mail transmission (if contact information is provided for the specific mode of delivery), or first-class mail, postage prepaid, to the designated representative below.
