Injury Reports. In the case of on-the-job injury, a copy of any worker's compensation claim filed by an employee shall be forwarded to the Union President, if the employee so requests, within seven (7) calendar days of that re- quest.
Injury Reports. 25.01 If an employee is injured at work, he or she must cause an accident report to be made electronically to a supervisor or administrator within twenty-four (24) hours of the event causing the injury. The Superintendent, the Superintendent’s designee or the Business Manager may require the employee to be examined by a physician or other health care provider of the administration’s choice, with the Board paying the cost of such exam. Injury Reports. The safety of all people in the library is important. Caution, care and common sense are critical to all work done in the library. Please ask for safety equipment, such as gloves or a step stool, if needed for a volunteer job. If an accident or unusual occurrence should happen, regardless of the severity, report the incident to a supervisor immediately. Evaluations Every volunteer will be evaluated at 3 months. This evaluation is to assess job training needs, to ensure the volunteer has been placed in a position that is suitable for him/her and the reevaluation of the volunteer’s schedule. This is an opportunity for the volunteers to express their likes and dislikes about being a volunteer. It’s an opportunity to assess job performance and whether the volunteer would like to continue his/her service. This is also an opportunity for the library to express any concerns they may have and to decide whether the volunteer service is still needed.

  • INCIDENT REPORTS Any serious occurrence involving a beneficiary, outside the normal routine of the OTP (see TRICARE Operations Manual (XXX), Chapter 7, Section 4), shall be reported to the referring military providers and/or Military Treatment Facility (MTF)/Enhanced Multi-Service Market (eMSM) referral management office (on behalf of the military provider), and DHA, and/or a designee, as follows: (a) An incident of a life-threatening accident, patient death, patient disappearance, suicide attempt, incident of cruel or abusive treatment, or any equally dangerous situation involving a beneficiary, shall be reported by telephone on the next business day with a full written report within seven days. (b) The incident and the following report shall be documented in the patient’s clinical record. (c) Notification shall be provided, if appropriate, to the parents, legal guardian, or legal authorities.

  • Daily Job Reports 6.7.2.1 Developer shall maintain, at a minimum, at least one (1) set of Daily Job Reports on the Project. These must be prepared by Xxxxxxxxx's employee(s) who are present on Site, and must include, at a minimum, the following information:

  • False Statements Contractor represents and warrants that all statements and information prepared and submitted by Contractor in this Contract and any related Solicitation Response are current, complete, true, and accurate.
