Individual Users. If you are an individual using the Services on your own behalf, you represent and warrant that: (1) you are at least 18 years of age and are of legal age to form a binding contract and (2) you have not previously been suspended or prohibited from using the Services. You may only use the Services and your CoinBurp Account for your own account and not on behalf of, or for the account of, any third party.
Individual Users. Each QHIN shall require that Individual Users with whom it has a Direct Relationship be identity proofed at a minimum of IAL2 prior to issuance of access credentials by the QHIN. The identity information may be supplemented by the Participant or Participant Member acting as authoritative sources by using knowledge of the identity of the Individuals in accordance with written policies and procedures. Such policies and procedures must be commensurate with the risk of incorrect identity proofing (e.g., procedures for applicants receiving credentials to access their medical information may be less rigorous than procedures used for applicants receiving credentials that can be used to access medical information on multiple patients). For example, IAL2 identity proofing for an applicant receiving credentials to access to his or her own medical information can be accomplished by any two of the following:

Individual Users. In case of individual user, we intend to provide our Service to an individual who is at least 18 years of age or more (or of legal age under other applicable laws). The User represents and warrants that he/she (a) is of legal age to execute and have full power and capacity to agree to and be bound by these Terms, (b) has not previously been suspended or rejected from using our Service, and (c) has not been sanctioned or restricted to use or transact through our Service or under any sanction list of any government or international organization.

Individual Users. You will pay Infonautics for all fees, charges, and any applicable taxes, associated with the Service in accordance with the current rates, payment terms, and policies specified by Infonautics in the applicable Service enrollment screens and on-line help. You acknowledge that you will pay Infonautics all such fees, charges, and applicable taxes for the Service where enrollment in the Service has been made using your credit card or other means, whether made by you personally or by someone else in your household or organization. If you enroll in the Service using a credit card, you certify to Infonautics that you are at least 18 years of age and that you are the cardholder of the credit card, or that you are expressly authorized by the cardholder to enroll in the Service. You further acknowledge that your obligation to pay Infonautics all such fees, charges, and applicable taxes for the Service does not depend on usage of the Service, and that you remain obligated to pay Infonautics for the Service regardless of whether the Service is used or not. Your obligation to pay for the Service begins on the date of your enrollment in the Service, not the date of your first use of the Service. If made available, free trial or free access periods to use the Service also begin on the date of your enrollment in the Service, and not the date of your first use of it. You may cancel your monthly or annual account for the Service during any free trial or free access period without charge. You may also cancel your monthly or annual account for the Service at any other time; however, Infonautics does not prorate any fees, charges, or applicable taxes associated with the Service upon any termination or cancellation and does not refund any such amounts. You may cancel your account for the Service by completing and submitting a cancellation form, and instructions for cancellation may be found in the on-line help for the Service.

Individual Users. User promises and warrants that User will obtain signed Activity Participation Agreements (either provided by or acceptable to Owner) from each participant in the activity. If the participants are minors, User will obtain the signature of at least one parent or legal guardian on each Activity Participation Agreement.
Individual Users. By accessing or using the Services in any way, you are representing that you have the authority and permission to enter into the Terms on your own behalf and to use the Services as contemplated hereunder, including without limitation any consent necessary to use the Services in connection with any Student with whom you use the Services.
