Common use of Individual Records Clause in Contracts

Individual Records. Pharmacy must maintain an individual record of administration including: individual’s name, address, telephone number, date of birth, allergies and gender; vaccine administered, the manufacturer, expiration date, lot number, site of administration, and dose administered; date of original order and the date of administration(s); name and address of Authorizing physician, and the name and address of Pharmacist administering the dose, and 5 N.J.A.C. 13:39-4.21(g),(h) & (k) * A pharmacist may administer an influenza vaccine to a patient under ten (10) years of age pursuant to a prescription by an authorized prescriber, N.J. Stat. 45:14-63. 6 N.J.A.C. 13:35-6.26; N.J.Stat. 45:14-63. 7 N.J.A.C. 13:39-4.21(i) the immunization location, if different from Pharmacy practice site; and name and address of the individual’s primary care provider, if provided.
