Individual Choice. In keeping with the standards and implementation specifications used by the FFE, Non-Exchange Entities should ensure that Consumers, Applicants, Qualified Individuals, Enrollees, Qualified Employees, and Qualified Employers, or these individuals’ legal representatives or Authorized Representatives, are provided a reasonable opportunity and capability to make informed decisions about the creation, collection, disclosure, access, maintenance, storage, and use of their PII.

Individual Choice. THE XXXXXX CENTER uses the concept of individual choice to assure that individuals are afforded the same choices that every member of the community have as their right. There are several areas regarding the choices individuals have been presented with that require further explanation here. Violation of any of these principles will result in THE XXXXXX CENTER removing a provider from the list of agencies given to individuals when choosing services and service providers.  No solicitation of individuals (or their families) currently being served by another provider is permitted. Such behavior is considered inappropriate and unethical.  No action will be taken to change the services for which an individual is authorized until that individual (or their guardian) has informed the individual’s Service Coordinator that they wish to make a change in services. Providers may not initiate changes in any individual’s CARE Code or service provider. Changes may only come from the individual and be authorized by the Service Coordinator and documented on a Person Directed Plan.   • Spouse The spouse of an eligible employee (if legally married under Minnesota law). For the purposes of health insurance coverage, if that spouse works full-time for an organization employing more than one hundred (100) people and elects to receive either credits or cash (1) in place of health insurance or health coverage or (2) in addition to a health plan with a seven hundred and fifty dollar ($750) or greater deductible through his/her employing organization, he/she is not eligible to be a covered dependent for the purposes of this Article. If both spouses work for the State or another organization participating in the State's Group Insurance Program, neither spouse may be covered as a dependent by the other, unless one spouse is not eligible for a full Employer Contribution as defined in Section 3A. Effective January 1, 2015 if both spouses work for the State or another organization participating in the State’s Group Insurance Program, a spouse may be covered as a dependent by the other.

  • Beneficiary The Participant may file with the Committee a written designation of a beneficiary on such form as may be prescribed by the Committee and may, from time to time, amend or revoke such designation.
