Implant Benefits. The implant and the associated crown over the implant are a benefit for persons sixteen (16) years of age and over, limited to once per tooth in a lifetime. Some implant procedures or procedures associated with implants are not covered services under the plan and no benefits will accrue or be payable for those excluded procedures.
