High Deductible Health Insurance Plan. The Village is offering, on a voluntary basis, the opportunity for Village employees to participate in a higher deductible health insurance plan (PPO2 Plan). In addition, the Village retains the right to introduce on a voluntary basis a high deductible health insurance plan.
High Deductible Health Insurance Plan. Under the Affordable Care Act (ACA) employers are required to provide affordable health care coverage for all employees. Given the cost of the 20% employee premium sharing for the City’s current health care plan which is mandated by State law there could be occasions where the City’s plan is deemed unaffordable for certain employees. The City and the Unions recognize and agree that it is in the best interest of the parties to comply with the ACA therefore, the parties agree to the following:
High Deductible Health Insurance Plan. ‌ Employees other than transportation employees who work more than four (4) hours per day and transportation employees who work more than four and ½ (4.5) hours per day shall be eligible to participate in the benchmark high deductible health insurance plan paired with a health savings account, effective January 1, 2018. For the three years of this Agreement, the Board will pay the full cost of the premium for this plan. Employees on an approved unpaid leave of absence may participate in the plan, but must pay both the employee and employer share of premium.

  • Group Health Insurance Immediately following retirement, the teacher shall have the option of remaining in the Corporation’s current group health insurance plan if all of the following conditions are met as of the date of retirement and thereafter:

  • Health Insurance Benefits To the extent provided by the federal COBRA law or, if applicable, state insurance laws, and by the Company’s current group health insurance policies, Executive will be eligible to continue Executive’s group health insurance benefits at Executive’s own expense. Notwithstanding the foregoing, if the Company determines, in its sole discretion, that it cannot provide the foregoing benefit without potentially incurring financial costs or penalties under applicable law (including, without limitation, Section 2716 of the Public Health Service Act), the Company shall in lieu thereof provide to Executive a taxable monthly amount to continue his group health insurance coverage in effect on the date of separation from service (which amount shall be based on the premium for the first month of COBRA coverage), which payments shall be made regardless of whether Executive elects COBRA continuation coverage and shall commence in the month following the month in which Executive incurs a separation from service and shall end on the earlier of (x) the date on which Executive voluntarily enrolls in a health insurance plan offered by another employer or entity during the period in which the Company is paying such amounts and (y) 12 months after the date of Executive’s separation from service.
