Health Promotion and Wellness Activities. The Contractor must provide a range of health promotion and wellness informational activities for Enrollees, their family members, and other significant informal caregivers. The focus and content of this information must be relevant to the specific health status needs and high-risk behaviors in the Medicare-Medicaid population. Interpreter services must be available for Enrollees who are not proficient in English. Examples of health promotion and prevention seminar topics include, but are not limited to the following:

Health Promotion and Wellness Activities. 2.5.7.1 Enrollee Health Education. The Contractor will offer an expansive set of health education programs, including such programs through Care Coordinators that use comprehensive outreach and communication methods to effectively educate Enrollees, and their families and other caregivers, about health and self-care and how to access plan benefits and supports.

Other Professional and Support Disciplines Consistent with the Enrollee‘s ICP, the Contractor may employ or contract with Community Health Workers under the supervision of the ICT to provide: Wellness coaching to engage the Enrollee in prevention activities such as smoking cessation, exercise, diet, and obtaining health screenings; Evidence-based practices and techniques for chronic disease self-management; Qualified peer support for Enrollees with mental health and substance use disorders to assist such Enrollees in their recovery, and for Enrollees with physical disabilities to assist such Enrollees in the pursuit of independent living; and Community supports for newly housed Enrollees who have experienced chronic homelessness. Community Health Workers must be available and appropriate for the populations served, such as for Enrollees who are Deaf or hard of hearing. Coordinating Services with Federal, State, and Community Agencies General Agencies. The Contractor must implement a systemic process for coordinating care and creating linkages between Enrollees and organizations that provide services not covered under the Demonstration, including but not limited to: State agencies (e.g. the Department of Developmental Services (DDS), Department of Mental Health (DMH), Department of Public Health (DPH) and DPH‘s Bureau of Substance Addiction Services (DPH/BSAS), Massachusetts Commission for the Blind (MCB), Massachusetts Commission for the Deaf and Hard of Hearing (MCDHH), Massachusetts Rehabilitation Commission, and the Executive Office of Elder Affairs (EOEA)); Social service agencies; Community-based mental health and substance use disorder programs; Consumer, civic, and religious organizations; and Federal agencies (e.g. the Department of Veterans Affairs, Housing and Urban Development, and the Social Security Administration). The ICDS Plan must develop and offer a range of health and wellness programs and informational materials that target specific health needs and risk behaviors identified for the ICDS Plan’s membership. At a minimum, the ICDS Plan must ensure that an assessment of the overall Beneficiary’s health needs is conducted and that the results are used to inform the development and/or refine of the health and wellness programs and informational materials. The CICO must provide a range of health promotion and wellness informational activities for Enrollees, their family members, and other informal caregivers. The focus and content of this information must be relevant to the specific health status needs and high-risk behavioral in the Medicare-Medicaid population. Interpreter services must be available for Enrollees who are not proficient in English. The CICO shall ensure that Providers have the preventive care, disease- specific and plan services information necessary to support Enrollee education in an effort to promote compliance with treatment directives and to encourage self-directed care.
