Health of Program Participants. College shall ensure that Program Participants meet all applicable local, state and federal health requirements. College shall provide to Hospital satisfactory evidence that each Program Participant is free from contagious disease and does not otherwise present a health hazard to Hospital patients, employees, volunteers or guests prior to his or her participation in the Program. Such evidence shall include without limitation the completion of a tuberculin skin test (within the last six months) or evidence that each Program Participant is free of symptoms of pulmonary disease if the skin test is positive, a chest x-ray following a positive TB test result, negative drug screening, physical examination, proof of rubella, and rubeola immunity by positive antibody titers or 2 doses of MMR, flu shot, and evidence of completion of the series of three hepatitis B vaccinations (if required by applicable law or Hospital policy). College and/or the Program Participant shall be responsible for arranging for the Program Participant’s medical care and/or treatment, if necessary, including transportation in case of illness or injury while participating in the Program at Hospital. College shall provide to Hospital proof of Health Insurance coverage for each program participant. In no event shall Hospital be financially or otherwise responsible for said medical care and treatment.

Health of Program Participants. All Program Participants shall pass a medical examination acceptable to Hospital prior to their participation in the Program at Hospital at least once a year or as otherwise required by Florida law. School and/or the Program Participant shall be responsible for arranging for the Program Participant's medical care and/or treatment, if necessary, including transportation in case of illness or injury while participating in the Program at Hospital. In no event shall Hospital be financially or otherwise responsible for said medical care and treatment. Program Participants will present the following health records on the first day of their educational experience at Hospital (Program Participants will not be allowed to commence experiences until all records):
Health of Program Participants. School will advise Program Participants that no Program Participant will be permitted to attend the Clinical Program until he or she submits to a medical examination acceptable to Hospital. School will require each Program Participant to maintain health insurance and provide proof of health insurance to the School prior to participating in the Clinical Program. School will cause Program Participants to provide to Hospital proof of health insurance as requested by Hospital from time to time. In the event a Program Student is exposed to an infectious or environmental hazard or other occupational injury while at the Hospital, the Hospital, upon notice of such incident from the Participating Student, will provide such emergency care as is provided its employees. School acknowledges that Hospital is not and will not be financially responsible for a Program Participant's medical care or treatment regardless of the Program Participant's condition or injury or cause of injury whether occurring at the Facility or otherwise and regardless of fault or cause of injury. School will ensure that each Participating Student furnishes to Hospital prior to each non- consecutive Rotation a complete copy of the following health records (Participating Students will not be allowed to access the Facility until all records are provided):

Such evidence shall include without limitation the initial completion of a two-step Tuberculin skin test. Thereafter, Tuberculin skin test every 12 months. Chest X-ray required following a positive TB Skin test result within the last 12 months or evidence that each Program Participant is free of symptoms of pulmonary disease. If the skin test is positive, a chest x-ray is required. Proof of negative drug screening. Proof of Varicella positive antibody titer or two doses of Varicella vaccine in the event of a negative titer. Proof of Measles, Mumps and Rubella (MMR) immunity by positive antibody titers or 2 doses of MMR, in the event of a negative titer. Evidence of a positive Hepatitis B antibody Titer or completion of the series of three Hepatitis B vaccinations in the event of a negative antibody titer (if required by applicable law or Hospital policy). Proof of Tdap Vaccination (Tetanus, Diphtheria, and Pertussis) within the last 10 years; and Influenza Vaccination required annually between the months of October and March, with extensions as required by the Center for Disease Control (CDC). Exemptions for Influenza Vaccination allowed only with Physician documentation and/or for religious reasons. School and/or the Program Participant shall be responsible for arranging for the Program Participant’s medical care and/or treatment, if necessary, including transportation in case of illness or injury while participating in the Program at Hospital. In no event shall Hospital be financially or otherwise responsible for said medical care and treatment.
