Common use of Health Insurance Clause in Contracts

Health Insurance. If Subsection (b) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separation, then the Company shall pay the Employee’s monthly premium under COBRA until the earliest of (i) the close of the Continuation Period, (ii) the expiration of the Employee’s continuation coverage under COBRA and (iii) the date when the Employee is offered substantially equivalent health insurance coverage in connection with new employment or self-employment.

Health Insurance. If Subsection (b) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“"COBRA”") following the Separationtermination of his Employment, then the Company shall pay the Employee’s 's monthly premium under COBRA until the earliest of (i) the close of the Continuation Period, (ii) the expiration of the Employee’s 's continuation coverage under COBRA and (iii) the date when the Employee is offered substantially equivalent health insurance coverage in connection with new employment or self-employment.

Health Insurance. If Subsection subsection (b) above applies, and if the Employee elects you elect to continue his and pay your health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separationtermination of your Employment, then the Company shall pay the Employee’s your monthly premium under COBRA until the earliest of (i) the close end of the Continuation Period, (ii) the expiration of the Employee’s your continuation coverage under COBRA and or (iii) the date when the Employee is offered you receive substantially equivalent health insurance coverage in connection with new employment or self-employment.

Health Insurance. If Subsection (b) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separationtermination of his Employment, then the Company shall pay the Employee’s monthly premium under COBRA until the earliest of (i) the close of the Base Continuation Period or Change of Control Continuation Period, as applicable, (ii) the expiration of the Employee’s continuation coverage under COBRA and or (iii) the date when the Employee is offered receives substantially equivalent health insurance coverage in connection with new employment or self-employment.

Health Insurance. If Subsection (ba) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) for the Employee and, if applicable, the Employee’s dependents following the Separationtermination of the Employee’s employment, then the Company shall pay the Employee’s employer portion of the monthly premium under COBRA for the Employee and, if applicable, such dependents until the earliest of (i) the close of the Continuation Period, (ii) the expiration of the Employee’s continuation coverage under COBRA and or (iii) the date when the Employee is offered receives substantially equivalent health insurance coverage in connection with new employment or self-employment.

Health Insurance. If Subsection (ba) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separationtermination of his Employment, then the Company shall pay the Employee’s monthly premium under COBRA until the earliest of (i) 6 months after the close Employee’s cessation of the Continuation Periodemployment, (ii) the expiration of the Employee’s continuation coverage under COBRA and COBRA, or (iii) the date when the Employee is offered receives substantially equivalent health insurance coverage in connection with new employment or self-employment.

If Subsection (b) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separationtermination of his Employment, then the Company shall pay the Employee’s monthly premium under COBRA until the earliest of (i) the close of the Continuation Period, (ii) the expiration of the Employee’s continuation coverage under COBRA and (iii) the date when the Employee is offered substantially equivalent health insurance coverage in connection with new employment or self-employment.

If Subsection (b) above applies, and if the Employee elects to continue his her health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separation, then the Company shall pay the Employee’s monthly premium under COBRA until the earliest of (i) the close of the Continuation Period, (ii) the expiration of the Employee’s continuation coverage under COBRA and (iii) the date when the Employee is offered substantially equivalent health insurance coverage in connection with new employment or self-employment.

If Subsection subsection (b) above applies, and if the Employee elects to continue his her health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separationtermination of her Employment, then the Company shall pay reimburse, subject to the Six Months Delay Period, if applicable, the Employee’s monthly premium under COBRA until the earliest of (i) the close of the Base Continuation Period or the Change of Control Continuation Period, as applicable, (ii) the expiration of the Employee’s continuation coverage under COBRA and or (iii) the date when the Employee is offered receives substantially equivalent health insurance coverage in connection with new employment or self-employment.

If Subsection (b) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“"COBRA”") following the Separationtermination of his Employment, then the Company shall pay the Employee’s 's monthly premium under COBRA until the earliest of (i) the close of the Base Continuation Period or Change of Control Continuation Period, as applicable, (ii) the expiration of the Employee’s 's continuation coverage under COBRA and or (iii) the date when the Employee is offered substantially equivalent health insurance coverage in connection with accepts new employment or (including self-employment) that offers health insurance coverage.

Health Insurance. If Subsection subsection (b) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“"COBRA”") following the Separationtermination of his Employment, then the Company shall pay reimburse, subject to the Six Months Delay Period, if applicable, the Employee’s 's monthly premium under COBRA until the earliest of (i) the close of the Base Continuation Period or the Change of Control Continuation Period, as applicable, (ii) the expiration of the Employee’s 's continuation coverage under COBRA and or (iii) the date when the Employee is offered receives substantially equivalent health insurance coverage in connection with new employment or self-employment.

If Subsection (b) above applies, and if the Employee elects to continue his health insurance coverage under the Consolidated Omnibus Budget Reconciliation Act (“COBRA”) following the Separation, then the Company shall pay the Employee’s monthly premium under COBRA until the earliest of (i) the i)the close of the Continuation Period, (ii) the expiration of the Employee’s continuation coverage under COBRA and (iii) the date when the Employee is offered substantially equivalent health insurance coverage in connection with new employment or self-employment.
