Health Care Professional. A physician or any of the following: podiatrist, optometrist, chiropractor, psychologist, dentist, Physician Assistant, physical or occupational therapist, therapist assistant, speech-language pathologist, audiologist, Registered or practical Nurse (including nurse practitioner, clinical nurse specialist, certified Registered Nurse anesthetist and certified nurse midwife), a licensed certified social worker, registered respiratory therapist and certified respiratory therapy technician.

Individually Identifiable Health Information (IIHI) – The “individually identifiable health information” as defined under HIPAA. Information Practices Act (IPA) – The California Information Practices Act, Civil Code § 1798 et seq. and the regulations issued pursuant thereto or as thereafter amended. If at any time you misrepresent yourself as a health care professional or cease to be a healthcare professional, then you will lose your certification immediately.

The Health Care Professional’s services must be rendered within the lawful scope of practice for that type of Provider in order to be covered under this Policy.

Health Care Professional a person licensed, certified, or registered to practice in a health care capacity by the appropriate licensing, certification or registration authority, as deemed appropriate by Us, in the jurisdiction where the care or services are provided or, where no such authority exists, has a certificate of competency from the professional body which establishes standards of competence and conduct for the profession, and is acting within the scope of that certificate. Hospital/Active Treatment Hospital: an institution which is legally licensed as a Hospital in the jurisdiction where the services are provided, for the care and Treatment of resident Inpatient or Outpatient. The term Hospital as used in this Agreement shall not include: rehabilitation hospital, rest facility, nursing home, convalescent home, health spa, hospice, clinic, or institution to treat substance abuse. Hospitalization: services rendered to a Participant confined to a Hospital. Illness: a deterioration of health or a bodily disorder that has been diagnosed by a Physician and requires regular and continuous care. Incidental Expenses: miscellaneous expenses incurred by the covered Inpatient as a result of Hospitalization due to a covered Accident or Illness (e.g. telephone, television). Inpatient: a Participant admitted and confined to a general Active Treatment Hospital for more than twenty-four (24) consecutive hours on the recommendation of the attending Physician.
