Health Benefit Program. The Plan’s applicable program that defines the health care services for which a Plan Member is eligible and the conditions and circumstances under which the Plan is obligated to pay for such services on behalf of the Plan Member.

Section 1 The City shall provide to Employees covered under this Agreement a Health Benefit Program which is provided for under the City's master group benefits agreements and shall be subject to the same deductible, co-insurance requirements, and other cost containment provisions as currently apply to these coverages. Health Benefit Program. Section 1. (A) All Employees covered under this Agreement shall continue to be covered by Horizon Blue Cross Blue Shield of New Jersey with the premium cost of the entire plan to be paid by the Employer. As part of the plan, employees shall continue to have a $50.00 co-pay for each emergency room visit and a $10.00 co-pay for each doctor’s visit, and in-network coverage shall continue to be 100%. Out-of- network coverage shall continue to be 80% and the annual out-of-pocket maximum shall continue to be $2,000 ($4,000 family); however, effective January 1, 2010, the annual out-of-network deductible shall be changed to $1,500 ($3,000 family), $2000 annual out of pocket maximum ($4000/family) and 60% coverage out-of-network.

A program which defines and limits the medical and Facility Services for which a Member is eligible, includes the conditions and circumstances under which the Health Plan is obligated to pay participating providers for Covered Services on behalf of Members and defines the Copayment Charges for which a Member is responsible. The Authority/CMCMUA shall provide medical coverage at not less than such coverages as the State of New Jersey may provide its employees, via the New Jersey State Health Benefits Program (NJSHBP) NJ DIRECT10-#050(1) Plan. The Authority/CMCMUA shall provide a prescription plan at no less coverage than the State of New Jersey provides to its employees, via the PRESCRIPTION DRUG PROGRAM #201 Plan. The Authority/CMCMUA shall provide a dental plan at no less coverage than the State of New Jersey makes available to its employees. The Authority/CMCMUA shall provide a vision plan equal to the plan in place as of January 1, 2013 as provided by UnitedHealthcare Vision (Spectera).

Health Benefit Program. Effective with the first pay period of January 2018, the City will contribute a maximum of $1568 per month for employee and dependent monthly medical, dental and vision premiums for insurance plans offered through the City. Employees will not receive any cash payout of extra monies.
