Health Benefit Plans. A. Upon ratification the employer health benefits contribution for each employee shall be an amount equal to eighty percent (80%) of the weighted average of the Basic health benefit plan premiums for a State active civil service employee enrolled for self-alone, during the benefit year to which the formula is applied, for the four Basic health benefit plans that had the largest State active civil service enrollment, excluding family members, during the previous benefit year. For each employee with enrolled family members, the employer shall contribute an additional eighty percent (80%) of the weighted average of the additional premiums required for enrollment of those family members, during the benefit year to which the formula is applied, in the four Basic health benefit plans that had the largest State active civil service enrollment, excluding family members, during the previous year. To be eligible for this contribution, an employee must positively enroll in a health plan administered or approved by CalPERS.
Health Benefit Plans. The College agrees to provide the following health benefit plans to all full time, probationary, continuous appointment and short-term appointment employees provided they meet the eligibility requirements of the Plans. The College agrees to contribute towards the premiums of said Plans in the following manner: Plan College Contributions for Full-time Employees Employee Contribution Medical Services Plan of B.C. Health Benefit Plans. The health and welfare benefits outlined in this Article apply to all unit members, including those hourly unit members teaching in the Alternative Programs and/or Schools.
Health Benefit Plans. When the enrollment and other requirements of the benefit carriers have been met, the Employer shall take steps to contract for and implement the following group plans:
Health Benefit Plans. .01 The Employer will use the best efforts to maintain the overall value of the Extended Health & Dental Benefits plan (Sun Life Financial Contract 4088, effective January 1, 2013) during the period of this Agreement. The Employer agrees not to amend the plans or premium cost sharing formula unless mutually agreed to by the Union and the Employer.

Health Benefit Plans. The Provider will cooperate to work within the City’s health benefit structure and with BISD’s self- insured benefit risk pool to assure proper services and referrals within the provider network and available benefits. This provision shall not restrict a healthcare provider from discussing all health care options with a patient. The Provider will verify enrollment in the City’s Health Plan or BISD’s self-insured benefit risk pool prior to treatment at the Center. The parties will cooperate to promote the use of the Center to Plan Members. All marketing, communication or professional publication referencing the Center must be reviewed and approved in writing by the City prior to distribution.
Health Benefit Plans. The Employer shall within ninety (90) calendar days of ratification of the collective agreement contract for and implement the following group plans for coverage of all eligible Employees and where applicable their dependents.
