Health and Welfare Funds. 16.01 The Employer will pay the amount as set out in Schedule A for all scheduled hours worked for each Employee towards the Union’s Health and Welfare Fund. No amount of these payments will be deducted from an employee’s wages. The total amount will be remitted to the Union 15 days following every 2nd pay cycle together with an itemized list of the employees for whom the remittances are made and the amount remitted for each.

Health and Welfare Funds. 18.01 The Employer shall contribute the amount set forth in the Schedule attached hereto to the Health and Welfare Fund. The said contribution shall be remitted in respect to each and every hour an employee works within each of the respective territories. Such contributions are to be made solely by the Employer and no Employer will deduct such contributions or any portion thereof from the Employee's wages. Such contributions (hereinafter sometimes called "Contributions") are in excess of wage rates set out in this Agreement and do not constitute a payment of wages or any portion of payment of wages. Health and Welfare Funds. The City shall contribute $23.70 per week to the Local 25 Health and Welfare Fund (for Dental and Vision Coverage only) for all employees who work 20 hours per week or more. Effective July 1, 2018, the amount contributed by the City will increase to $24.70 per week. Effective July 1, 2019, the amount contributed by the City will increase to $25.90 per week. These amounts represent $13.00 plus 50% of the cost of the plan in excess of $13.00. The weekly employee contributions will be as follows: $10.70 effective July 1, 2017; $11.70 effective July 1, 2018; $12.90 effective July 1, 2019.

A Board of Trustees has been created consisting of an equal number of members, appointed by the Association and an equal number appointed by the Local Union. The duties of this Board of Trustees shall be to execute and administer the Declarations of Trust in accordance with Federal, State and Local laws. Said Welfare Fund shall continue from year to year unless terminated by either party hereto giving unto the other party ninety (90) days written notice prior to April 30th of the particular year of its intention to terminate same, so long as the Collective Bargaining Agreement between the parties hereto shall provide for the continuance of the Industry Advancement Fund referred to in Section 1, hereof. It shall be the responsibility of the Trustees to determine eligibility of anyone receiving benefits.

The detailed basis of the administration of the Health and Welfare Fund shall be pursuant to the Agreement and trust declarations adopted by the Associations, Independent Floor Covering Employers and the Union, which shall be binding upon all Employers signatory to or bound by this Agreement.

The weekly employee contributions will be as follows: $14.10 per week effective July 1, 2020 and $15.50 per week effective January 1, 2021.
