Health and Welfare Contributions a. For each day in which a recording musician performs work under this Agreement, the Employer shall contribute $25.00 (twenty-five dollars) on behalf of each musician so employed. The Employer shall also contribute an amount calculated in accordance with Section IV on behalf of those musicians employed under a page rate (i.e. Orchestrators, Copyists and other Music Preparation).
Health and Welfare Contributions. Effective July 1, 2007 the County shall contribute a total of $7,550 per year, effective July 1, 2013 a total of $8050 per year and effective July 1, 2014 a total of $8,550 per year toward employee health and welfare coverage. Employees who work between 4 and 6 hours may choose not to participate in the County provided medical coverage. These employees may choose to receive cash-in-lieu as follows:

The Company would maintain health and welfare benefits at current levels by maintaining current contributions, and increasing them by up to the following amounts, if and as required by the applicable health and welfare funds: January 1, 2015 up to $.35/hr August 1, 2015 up to $.50/hr August 1, 2016 up to $1.00/hr August 1, 2017 up to $1.00/hr August 1, 2018 up to $1.00/hr The 2010 Restructuring Agreement would be modified so that the August 1, 2014 health and welfare increase will be deferred to January 1, 2015, as reflected above.

The following is descrip- tion of the plans and the insurance policies should be consulted for a comprehensive description of the coverage provided. The Company to provide to its employees the Health and Welfare Plan described in herein. Employees will be eligible to become members of the plan upon completion of Forty-Five calendar of continuous with the Com- pany. provided are at work on that date. If not at work on that date. they will eligible immediately upon return to work.

The following is a description of the plans and the insurance plan text that should be consulted for a comprehensive description of the insurance coverage period. The Company agrees to provide to its employees the Health and Welfare Plan described in Schedule "A" herein. Employees will be eligible to become members of the plan upon completion of two with the Company, provided they are at work on that date. If not at work on that date, they will be eligible immediately upon return to work.
Health and Welfare Contributions. The Employer and the Union by executing this Agreement agree to be bound by all provisions of the Agreements of Trust, including any modifications or amendments thereto. It is agreed that the above obligations exist without the necessity of executing any additional written instrument. EXHIBIT “C” - PENSION CONTRIBUTIONS
Health and Welfare Contributions. The Employer shall contribute to the Health and Welfare Fund in accordance with Article XII of the main body of this Agreement for employees who work thirty (30) or more hours per week. Health and Welfare contributions shall not be required for employees working less than thirty (30) hours per week unless required by law.

Casual Employees— The Employer shall pay the sum designated below for each day worked by a casual employee covered by this Agreement, into the Chicago Truck Drivers, Helpers and Warehouse Workers Union (Independent) Health and Welfare Fund here tofore created by an Agreement and declaration of trust pursuant to collective bargaining agreements between certain Employers and the Union created for the payment of health and welfare benefits to covered employees, and the Union agrees to provide health and welfare benefits to each employee with respect to whom the Employer makes such payment as follows: Effective 7 /1 /73 : $3.80 per day worked with a maximum of $19.00 per week, and so forth from there on, in accordance with the agreements signed between the Union and the Central Motor Freight Association. Days off which are paid for under the holiday and vacation provisions of this Agreement shall be con sidered as days worked for the purpose of this Section. The Employer shall transmit the contributions due hereunder with respect to each month not later than the 10th of the following month. Contributions will continue during any period a casual employee is absent because of an occupational illness or injury covered by tbe Workmen’s Compen sation Law for whicbi the Employer is liable for a period of twelve (12) months or until the employee returns to work, whichever period is the shorter. The Employer further agrees to make the above contri butions for casual employees during periods when a collective bargaining agreement is being negotiated.
