GROUP INSURANCE BENEFIT. A. Medical Plan The Board agrees to provide Blue Shield PPO Blue Qualified High Deductible Health Plan, the specifics of which are outlined and attached in Exhibit A, for all members, including their spouses and dependent children. In the event any of the aforementioned is discontinued by the insuring company, the Board will provide similar or revised coverage equivalent to the above.
GROUP INSURANCE BENEFIT. The Employer will pay one hundred (100%) percent of the premium costs a two times annual earnings Group Life Insurance Plan for all employees working on a full-time basis after the completion of three months of continuous employment. The Employer will pay seventy-five percent (75%) of the premium costs for an Extended Health Care Plan effective August (deductible per individual and per family; no CO- insurance; maximum benefit per year per individual) for all employees working on a full time basis after the completion of three months of continuous employment. Effective January the Extended Health Benefits Plan will provide Vision Care Coverage (maximum every months). The complete terms and conditions of these protections are set forth in the Policies, Certificates and/or Plan of each of the insuring companies. The Employer's towards the insurance program will cease when an employee is laid off or terminated or on a leave of absence as specified in Article and Article of this collective agreement. The Employer shall pay fifty percent of the premium costs for a basic dental plan similar or the same as the Blue Cross Plan plus Rider current rates, the requirements of the Insurance Company are met. The employer shall pay fifty percent (50%) of the premium costs for additional dental benefits or the same as the Blue Cross Rider Level providing orthodontics for each insured child between six (6) and eighteen years of age, providing co-insurance, to a lifetime maximum of if the requirements of the insurance company are met. The employer shall pay fifty percent (50%) of the premium costs for additional dental benefits or the as the Blue Cross Rider Level providing the creation and repair of crowns and bridges, providing co-insurance, to a annual maximum per insured employee and dependant, if the requirements of the insurance company are met. Note: Co-insurance means that one half of the of expenses under the current Ontario Dental Association rates are reimbursed up to the specified maximum. Premium deduction for this benefit will commence in July, The Employer agrees to continue to extend the benefits of the Pension Plan presently in effect to all eligible employees in the bargaining unit.   • Continuation of Insurance Benefits Notwithstanding any other provision in this Agreement to the contrary, the Bank and/or its successor shall maintain in full force and effect for Employee's continued benefit, for the two (2) year period beginning upon a Change in Control, all life insurance, medical, health and accident and disability policies, plans, programs or arrangements which were in effect immediately prior to the Change in Control.
