Group Benefit Plan. 34.01 A group benefit plan (medical, dental and group life insurance) is provided to eligible employees on the terms set out in the plans. Detailed information on the group benefit plan is available upon request from the Employer’s Administration Office. Each employee upon enrollment in the group benefit plan shall receive a copy of the booklet outlining benefit coverage and eligibility. The Employer agrees to consult with the Union prior to making any changes to the group benefit plan.

Group Benefit Plan. ‌ The Group Benefit Plan Booklet will provide you additional information on the benefit entitlement identified in this article. The Extended Health Benefits, Group Life, Short-Term Disability and Long-Term Disability insurance, as well as the Dental Plan, will all be paid in accordance with the schedule of benefits listed in the carrier's plans, and subject to the limitation and eligibility requirements specified in the Plans. A current copy of the Group Benefit Plan booklet is available on the Xxxxxxx College website.

Short-term disability benefits are provided for employees following successful completion of the probationary period and shall start on the first (1st) day of absence due to an accident and the fourth (4th) day of absence due to illness. A summary of the Plan is provided upon enrolment. The Employer shall not be responsible for any benefit claims that the insurance carrier has determined to be non-qualifying claims and shall not be subject to the grievance and arbitration procedures of this Agreement.

Group Benefit Plan. (a) The Employer agrees to participate with employees in the provision of a Group Benefit Plan, which may not include Long-Term Disability coverage. If the Group Benefit Plan is different than the Plan as exists at the coming into force of this Agreement, it shall not be implemented without the approval of the Union. With the exception of the Health and Dental Plan components, the Employer agrees to pay fifty per cent (50%) of the total premium cost for all employees covered by the Group Benefit Plan. The Employer agrees to cost share on the basis of 65% of the premiums for the Health and Dental Plan components of the Group Benefit Plan for those Employees who are eligible and who pay their respective share on the basis of 35% of the premiums to participate in the Health and Dental Plans.
