The Staying Connected Program (the Program) is a call check-in service and notification service provided by the Ohio Department of Aging for Ohio residents age 60 or older. The Program will provide a daily call, during the same one-hour time period every day, to a qualified participant in the Program (Participant). These will be automated calls. If no one answers any of these calls, the Program will call an alternate contact person who has been selected by Participant (Alternate Contact Person). If the Program is unable to contact a Participant or leave a message with Participant’s Alternate Contact Person, the Program will notify Participant’s local non-emergency service.

The PPS Board of Education has specified that the District will ensure apprenticeship opportunities in the construction trades and will promote construction employment opportunities for people of color and women (ref. Equity in Purchasing and Contracting Policy 8.50.095‐P). This objective includes a) ensuring that the District does business with contractors whose workforce reflects the diversity of the workforce found in the city of Portland and Multnomah County, and b) that their contracting dollars provide fair and equal opportunities to the Districts’ diverse populations. To implement this objective, the District has engaged City of Portland, Procurement Services to provide and administer the Workforce Training & Hiring Program ("Workforce Program"). The Workforce Program applies to all projects estimated at $200,000 or more and to each subcontractor having a subcontract of $100,000 or more on the project. The Contractor and all subcontractors are encouraged to fulfill the program requirements even if their contracts are less than these amounts. Contractors shall make reasonable efforts to ensure that their workforce reflects the diversity of the city of Portland and Multnomah County. SAMPLE One way contractors can make reasonable efforts to ensure that their workforce is diverse is to recruit, train and employ minorities and women whenever possible. This portion of the Contract establishes requirements regarding that recruitment, training and employment. Contractor shall develop and operate a juvenile service, counseling, and diversion program to complement the current expansion of the Sheriff Department’s Juvenile Services Bureau. This Program provides Member with a user-friendly solution for developing, maintaining and hosting a professional-looking website that incorporates and displays the Touchstone Energy® imagery. Your Program account includes: ▪ Access to build and host one (1) website with the Program platform; ▪ One site build (optional, at the Member’s request), which involves the transfer of the pages and content from your existing site to a Program site; ▪ Hosting of the Program site; ▪ Access to Program training webinars, user manuals and other learning resources that we make available from time to time, which are also subject to change from time to time; ▪ Access to Touchstone Energy®’s professional Drupal support team during regular business hours (Monday through Friday, 9:00 a.m. to 5:00 p.m. eastern time) for assistance with design, support and training; and ▪ Access to make updates to the Program site on a 24/7 basis, from computers and mobile devices. The Program to be undertaken by NRMN will involve the purchase of bicycles, purchase of Facilities and their installation and obtaining authorization to construct and install the Facilities upon both public and private property to enable members of the public to rent bicycles for use and return to the Facilities.

Under this component of RAD, PHAs may choose between two forms of Section 8 Housing Assistance Payment (HAP) contracts: project-based vouchers (PBVs) or project-based rental assistance (PBRA). No incremental funds are authorized for this component. As such, initial contract rents are established based on public housing funding levels, and are subject to applicable program rent caps. Applications may be submitted for a specific project (using the RAD Application), a PHA-defined portfolio of projects, or a multi-phase project. If a PHA applies for either a portfolio or multi-phase award, HUD will reserve RAD conversion authority for the projects or phases covered by the award, and the PHA will be required to submit a RAD Application for each individual project or phase. Following review and selection of a RAD Application, HUD will provide the PHA with a Commitment to enter into a Housing Assistance Payment (CHAP), after which the PHA will have to present a Financing Plan for HUD to approve. After HUD approval of the Financing Plan and successful closing of the conversion, a project will receive a long-term Section 8 HAP contract. Upon conversion, units whose assistance has been converted pursuant to RAD will be removed from the public housing program and Covered Projects will be released from the public housing Declaration of Trust (DOT) and the Declaration of Restrictive Covenants (DORCs), if applicable.

This Program provides Touchstone Energy member cooperatives with a user-friendly solution for developing, maintaining and hosting a professional-looking web site that incorporates the Touchstone Energy brand trademarks, copyrights and other elements of its intellectual property portfolio as identified, set forth, consistent with and subject to the applicable Touchstone Energy Distribution License Agreement or the Touchstone Energy G & T / Regional Member License Agreement entered into between Touchstone Energy and its member cooperatives. Through the Program, Touchstone Energy member cooperatives have access, based upon the Cooperative’s elected support tier, to certain web site templates, website development services, and a web site content management system (together, "Web Development Services") and web site hosting services ("Hosting Services") provided by third party service providers. Also, depending upon the Cooperative’s elected support tier, Touchstone Energy provides certain support and development assistance consistent with this Agreement and the attached Exhibits A & B (“Support Services”).
