The nurse shall be provided appropriate forms for confirming return to work upon commencement of the leave. A nurse who fails to comply with the leave of absence procedures set forth in this Article, or who fails to return to work on the next day following termination of a leave of absence, is subject to termination of seniority and employment unless the nurse is prevented from returning to work for reasons beyond the nurse's control.
