Food Allergies. Senior and parent/guardian agree to abide by the Grad Nights life threatening food allergy policy and will provide an Emergency Care Plan if senior has a life threatening food allergy. Additionally, senior and parent/guardian agree and acknowledge that the Release of Liability and Indemnification provisions of this Agreement include claims and damages arising out of senior’s consuming or handling of the food provided during the Graduation Event.

Food Allergies. It is your responsibility to identify any food allergies of your guests and report them in writing to Hickory Heights Golf Club or have them identified on the Special Function Sheet. You agree to indemnify, defend and hold harmless Hickory Heights Golf Club from and against all liability, loss or damage, including reasonable attorneys’ fees and court costs which it incurs for any injury sustained to you, your guests or our staff resulting from a food allergy which was not disclosed to Hickory Heights Golf Club in writing and before your event.
Food Allergies. Some children may have life-threatening food allergies. To keep these children safe while at school, all parents and students must follow guidelines established by Xxxxxx the Xxxx Catholic School. These guidelines do not limit what other children can bring to eat at lunch, but are designed to allow the food allergic child to safely participate in all school activities. All classmates of food allergic children will receive a summary of these guidelines. At school-sponsored parties (Halloween, Christmas, etc.), effort should be made by the Room Mothers and other parents, with the cooperation and assistance of the food allergic child’s parent, to provide snacks that are safe for the entire class. FIRE DRILLS/TORNADO DRILLS/SAFETY DRILLS Xxxxxx the Xxxx Catholic School conducts fire, tornado, and safety drills during the course of the year in compliance with the State of Texas. Evacuation routes are posted in each room. Students shall move to designated areas quickly and in silence. In the case of an actual emergency, the students will be moved to a safe location. Parents should NOT come to the school until notified through RenWeb’s Parent Alert System to do so. The school cannot release a student until the authorities lift the warning or alert. FORMS Permanent, accurate and updated records of health information are kept on all students. All student files include the following: Emergency Release forms, Health History information, and Immunization Records. After these forms are turned into the office, parents will be contacted if their child needs an immunization, etc. Returning students need to inform the office of any updated information from their physician. It is necessary for parents to fill out a new Emergency Release for EACH child in the family enrolled in the school.
Food Allergies. 1. The school does not guarantee elimination of any and all food products that may cause the student to have an allergic reaction.
Food Allergies. The district requests to be notified when a student has been diagnosed with a food allergy, especially those allergies that could result in dangerous or possibly life-threatening reactions either by inhalation, ingestion, or skin contact with the particular food. It is important to disclose the food to which the student is allergic, as well as the nature of the allergic reaction. Please contact the school nurse if your child has a known food allergy or as soon as possible after any diagnosis of a food allergy. The district has developed and annually reviews a food allergy management plan, which addresses employee training, dealing with common food allergens, and specific strategies for dealing with students diagnosed with severe food allergies. When the district receives information that a student has a food allergy that puts the student at risk for anaphylaxis, individual care plans will be developed to assist the student in safely accessing the school environment. The district’s food allergy management plan can be accessed in the nurse’s office.
Food Allergies. If your child has a food allergy, you must notify us in writing so that we can make appropriate substitutions. The written notification should list appropriate food substitutions and must be updated at least annually. Food allergies can be life threatening and each child with a food allergy should have an action plan for emergency care completed by the family physician.‌

Substitutions will be made for students that have special dietary needs or food allergies, only when proper physician documentation and recommendations have been submitted to the school. Food Allergies. Participant has an obligation to notify Verdello of any food allergy prior to the event. While Verdello will attempt to reasonably accommodate a Participant’s allergies, participants will be exposed to various food products and Verdello cannot guarantee that Participant will not be exposed to potential allergens. If a potential Participant has any questions regarding a class and the ingredients of a class prior to the event, it is Participant’s sole responsibility to contact Verdello at 000-000-0000 during store hours. Participant hereby acknowledges the inherent risks associated with participating in a cooking class at Verdello with respect to potential allergens as described above and hereby assumes full responsibility for personal injury to his/herself and (if applicable) his/her family members. Participant further releases and discharges Verdello for injury, loss or damage arising out of his/her or his/her family’s exposure to allergens during a cooking class at Verdello, whether caused by the fault of Participant, his/her family, Verdello or other third parties.
