Field Records. Field notebooks and field photography; photo identification sheet will accompany 35 mm negatives and contact sheet; no archival requirements; submitted field records must fit into 9.5” x 12” folding file.
Field Records. If technical direction of the installation is called for under this Contract, Contractor’s representative shall maintain at the site, up-to-date copies of all drawings, specifications, and other contract documents and supplementary data, complete with latest revisions thereto. The Contractor shall maintain a continuous record of all field changes and, at the conclusion of the work, shall incorporate all such changes on the drawings and other engineering data and shall submit the required number of copies thereof to LES.

In addition, the Contractor shall maintain a continuous record of all field changes and, at the conclusion of the Work, shall incorporate all such changes on the drawings and other engineering data and shall submit the required number of copies thereof to CPS Energy. CPS Energy personnel shall be allowed to examine field records upon request; review of these records is not an endorsement or approval by CPS Energy as to adequacy or composition. Furthermore, such review shall not be considered as an assumption of any risk or liability by CPS Energy or any officer, agent or employee thereof. The Contractor shall also maintain at the Contractor’s site office copies of current safety-related documentation and records that include, but are not limited to: Contractor’s training and safety program and manuals; and Contractor’s training program schedule consisting of dates, times and personnel attending. The Contractor shall maintain at the Contractor’s field office a record of equipment used in performance of the Work herein requiring certification (i.e., cranes, aerial lifts, personnel platforms, etc.). This record shall contain a listing of the certifying agency, whether Federal (which has U.S. Department of Labor certification), State, local or private, by name, address, and phone number.
Field Records. The field forms used to develop a written record of assessment activities will include the following (examples are included in Appendix D): ■ Daily Field Reports; ■ Field Logbook; ■ Air Sample Analysis Forms; ■ Sample Custody Records; and, ■ Daily Sign-In Log A HASP prepared to address site-specific hazards is included as a separate document. The HASP will be reviewed and signed daily by field personnel prior to fieldwork, indicating that they understand the plan and its requirements. A copy of the HASP will be maintained onsite and made available to personnel during field activities. Further information regarding health and safety considerations, including special protective equipment (PPE) requirements will be included in the HASP.

In addition, Contractor shall maintain a continuous record of all field changes, which shall be available for inspection by Owner at all times. At the conclusion of the Work, Contractor shall deliver a copy of the continuous record to Owner, and shall incorporate all such changes on the drawings and other engineering data, and submit the required number of copies thereof to Owner.
Field Records. Owner and Contractor shall establish a system of daily report forms to be used to monitor all major equipment and support personnel which are chargeable at an hourly or a daily rate basis. All daily report forms will be signed by a field representative of Contractor and the Superintendent at the end of each shift. Such forms shall be maintained in a business-like and legible condition, subject at all times to audit by Owner's representatives.
