Eye Examination. 294. All employees who are Health Service System members shall be eligible for one (1) annual eye examination and prescribed eyewear for computer use.
Eye Examination. (a) When a majority of an employee's daily work time requires monitoring such VDT's, such employees shall have their eyes examined by an ophthalmologist of the employee's choice at the nearest community where medical facilities are available, and after six (6) months a further test, and annually thereafter, if the employee so requests. The examination shall be at the Employer's expense where costs are not covered by insurance. Time off work to attend the eye test shall be granted with pay.

Eye Examination. 10/5/1 If an eye examination for safety glasses is necessary, the Employer will pay the entire cost of one examination during the life of this Agreement.
Eye Examination. Effective June 1, 2006, the Employer shall pay the full cost of one (1) eye examination every twenty-four months (24) in addition to the aforementioned maximum amounts for vision care.
Eye Examination. To provide an eye examination up to $90 per family member per 24 months in each year of the Agreement. Where an employee is required to have additional eye examinations due to other medical reasons, the Company will pay the cost up to a total of $500.00 for every twenty-four (24) months.
Eye Examination. The Company agrees to pay the cost of eye exams to a maximum of $150.00 per employee, every 2 years, effective October 1, 2005. This benefit recommences every 2 years thereafter.
Eye Examination. Parents/guardians are encouraged to have their children undergo an eye examination whenever health examinations are required. Parents/guardians of students entering kindergarten or an Illinois school for the first time shall present proof before October 15 of the current school year that the student received an eye examination within one year prior to entry of kindergarten or the school. A physician licensed to practice medicine in all of its branches, or a licensed optometrist, must perform the required eye examination. If a student fails to present proof by October 15, the school may hold the student’s report card until the student presents proof: (1) of a completed eye examination, or (2) that an eye examination will take place within 60 days after October 15. The Director or designee shall ensure that parents/guardians are notified of this eye examination requirement in compliance with the rules of the IDPH. Schools shall not exclude a student from attending school due to failure to obtain an eye examination.
Eye Examination. Charges incurred by a Covered Person for one eye examination by a Duly Licensed or every consecutive months, up to a maximum of VISION CARE When prescribed by a Duly Licensed ophthalmologist or optometrist and dispensed by a Duly Licensed ophthalmologist, optometrist or optician, charges incurred by a Covered Person for laser eye surgery, eyeglass frames and corrective lenses, contact lenses, prescribed sunglasses and repairs to frames and corrective lenses will be covered, up to the maximum shown in the Summary of Coverages. Charges will be considered incurred on the date the Covered Person incurs an obligation with the provider for eligible vision care expenses. Benefits are not payable for: Safety glasses, whether prescribed or not; Non-corrective glasses whether prescribed or not.
