Eye Care Plan. The City will provide a $10 deductible eye care plan for employees. This will be an insured plan and is subject to the terms of the plan sponsor. This benefit is available to all IBEW full time employees. Employees who are assigned to positions which require safety glasses for work are to specify safety eye wear with side xxxxxxx when purchasing glasses to be worn at work.

Eye Care Plan. During any (24) consecutive months, eyeglass lenses, contact lenses eligible up to amount that would be charged for a pair of regular eyeglasses, except that this limit will not apply if contact lenses are the only way to restore visual acuity of better eye to at least or are acquired cataract surgery; eyeglass will not exceed the maximum amount payable of one hundred and fifty dollars ($150) and they are limited to one pair per family member. Sunglasses or safety glasses of any kind are excluded. Supplies must be prescribed in writing by an ophthalmologist or a licensed optometrist and must be dispensed by an ophthalmologist, a licensed optometrist or a qualified optician.

Maximum eye care reimbursement not to exceed $400.00 per year. Hearing devises are also covered with the understanding that coverage is for glasses or hearing devises.

Eye Care Plan. Amend Article XIII(B)(3) as follows: Effective July 1, 2005, the eyeglasses benefit will increase by $5.00 pursuant to the current bi-annual formula.
Eye Care Plan. Full-time employees and eligible dependents shall be eligible for the State-administered Eye Care Program. The program shall provide for each eligible employee and dependents to receive a $35.00 payment for prescription eye glasses with regular lenses and a $40.00 payment for such glasses with bi- focal lenses. Each eligible employee and dependent may receive only one payment during the two (2) year period that this program will be in effect. The extension of benefits to dependents shall be effective only after the employee has been continuously employed for a minimum of sixty (60) days. Full-time employees and eligible dependents, as defined above, shall be eligible for a maximum payment of $35.00 or the cost; whichever is less, of an eye examination by an Ophthalmologist or an Optometrist. Each eligible employee and dependent may receive only one payment for glasses and one payment for examinations during the period from July 1, 1999 to June 30, 2001 and one payment for glasses and one payment for examinations during the period July 1, 2001 to June 30, 2003. This program ends June 30, 2003.
