Extended Health Care Plan. (a) The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan.
Extended Health Care Plan. The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan. In addition to the eligible dependents covered in the existing plan, such Plans shall include coverage of both same sex and opposite sex common-law spouses. The Employer will amend its Extended Health Benefit Plan provisions to provide the following:
Extended Health Care Plan. The Employer shall pay the monthly premium for regular Employees entitled to coverage under a mutually acceptable extended health care plan. Coverage for emergency out-of province coverage will be at one hundred percent (100%), subject to a lifetime maximum of two hundred and fifty thousand dollars ($250,000).
Extended Health Care Plan. The Board shall pay one hundred per cent (100%) of the premium cost of the Provincial Extended Health Benefit Plan for each full and part-time teacher employed by the Board.
Extended Health Care Plan. (a) The Employer shall pay the monthly premiums for extended health care coverage for employees and their families under the Pacific Blue Cross plan. The maximum lifetime amount payable per eligible employee or eligible dependant shall be unlimited.
Extended Health Care Plan. The Commission shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan.
Extended Health Care Plan. G.1.6.1 The Employer shall contribute one hundred percent (100%) of the premium cost of an Extended Health Care Plan with a calendar year deductible feature of $25 per individual and $50 per family for all eligible full-time Employees who have enrolled in coverage under the Plan.
Extended Health Care Plan. A) The Extended Health Care Plan shall cover eligible employees, their spouses (including common-law spouse), and dependent children, provided they are not enrolled in another plan.
Extended Health Care Plan. S.5 The Employer shall provide an Extended Health Care Plan for eligible Employees which will include payment for eligible expenses as currently provided by the Board’s Extended Health Care Benefit.
Extended Health Care Plan. This plan is an insured plan providing for limited payments for additional medical benefits not covered by the Ontario Health Insurance Plan. The total premium of this plan is paid by the University of Waterloo. An employee and the University share the cost of eligible benefits under this Plan on a 20% / 80% basis respectively. Maximum dollar benefits may be applicable and there are maximum annual costs (caps) to the employee’s out of pocket expenses for eligible benefits under the Plan. Maximum dollar benefits and caps are subject to adjustment annually by the Pension and Benefits Committee.
