Extended Health Care i) Effective July 1, 2004 the annual Extended Health Care deductible will be increased to fifty dollars ($50.00) for single or family coverage.

Extended Health Care. The Hospital shall contribute on behalf of each eligible employee seventy-five percent (75%) of the billed premium under the Extended Health Care Plan (Liberty Health $15-25 deductible plan including hearing aids with a maximum of $300.00 per person and vision care with a maximum of $150.00 every 24 months per person, or its equivalent) provided the balance of the monthly premium is paid by employees through payroll deduction. The Extended Health Care Plan shall be amended to provide for a prescription drug dispensing fee cap of $9 per prescription. The Extended Health Care Plan shall be amended to provide for mandatory generic drug substitution. * Effective January 1, 2010, the Extended Health Care Plan shall be amended to provide for a vision care maximum of $300 per insured person every twenty-four (24) months. **Effective January 1, 2010, the Extended Health Care Plan shall be amended to provide for one (1) optometry exam every twenty-four (24) months (up to a $100 maximum).
