Common use of Establishment of Service Clause in Contracts

Establishment of Service. 6.1 After receiving certification as a local exchange carrier from the applicable regulatory agency, State Discount Telephone LLC will provide the appropriate BellSouth account manager the necessary documentation to enable BellSouth to establish accounts for resold services ("master account"). State Discount Telephone LLC is required to provide the following before a master account is established: proof of PSC/PUC certification, the Application for Master Account, an Operating Company Number ("OCN") assigned by the National Exchange Carriers Association ("NECA") and a tax exemption certificate, if applicable.
