Equipment Insurance. If you are leasing Equipment, You shall procure and continuously maintain and pay for (i) all risk insurance against loss of and damage to the Equipment for not less than the full replacement value of each Unit, naming us as loss payee, and (ii) public liability and property damage insurance insuring against third party personal and property damage in respect of the use and operation of the Equipment in an amount not less than 1,000,000 USD per occurrence, (iii) Each insurance policy shall name us as an additional named insured and a loss payee and provide that there shall be no recourse against us for payment of premiums or other amounts. The insurance shall be in such form and with such company or companies as shall be reasonably acceptable to us. You shall provide to us a certificate evidencing such insurance. You hereby appoint us as your attorney-in-fact to make claim for, receive payment of, and execute and endorse all documents, checks, or drafts received in payment for any loss or damage to the Equipment under any such insurance policy.
Equipment Insurance. The insurance shall be in such form and with such company or companies as shall be reasonably acceptable to us. Promptly, but in no event later than thirty (30) days from the Purchase Date, you shall provide to us a certificate evidencing such insurance. You hereby appoint us as your attorney-in-fact to make claim for, receive payment of, and execute and endorse all documents, checks, or drafts received in payment for any loss or damage to the Equipment under any such insurance policy. Failure to provide such certificate of insurance in a timely manner will be considered a breach of this Agreement.
Equipment Insurance. Contractor must secure, pay for, and maintain all-risk insurance as necessary to protect the City against loss of owned, non-owned, rented or leased capital equipment and tools, equipment and scaffolding, staging, towers and forms owned or rented by Contractor or its Sub-contractors.

Customers receive Roadside Rescue Services by Dialing #ROAD from their Sprint PCS phone when on the Sprint PCS network, dialing 000-000-0000 from any working phone when outside the Sprint PCS network, or while roaming on a Sprint PCS roaming partner network the customer may dial 000-000-0000 from their Sprint PCS dual band phone.

Equipment Insurance. Contractors equipment insurance covering equipment and tools, owned, rented or leased, for the full cost of replacement of such equipment on an "all risks" basis.
Equipment Insurance. User is required to purchase Equipment insurance in the amount of $50.00 which will cover the replacement cost of a stolen device, or repair or replacement of the device in the case of a malfunction or unintentional damage. In addition to the initial premium to purchase the insurance, for each covered claim, User shall pay a $50.00 deductible, or the actual cost of repair, whichever is less. Insurance does not cover theft from an unattended vehicle, except when it is securely locked, its windows are fully closed and there is visible evidence that entry into the vehicle was forced. In addition, the Insurance will not cover water damage, or any loss or damage to the Equipment where the only proof of loss is unexplained or is caused by the disappearance of the Equipment without the knowledge as to place, time or manner of its loss. If any Equipment is stolen, User is required to immediately notify local law enforcement and file a report. A copy of the police report must be submitted to the School along with any claim for theft. Insurance does not cover any claim for any loss, damage, theft or malfunction of accessories issued to the User, if any. Accessories include but shall not be limited to: cases, charging devices, charging cables, keyboards, mouses, etc., and are the sole responsibility of the User. Acknowledgment: By signing below, User acknowledges that they have read this Agreement, understand it, and agree to be bound by its terms and conditions. User further acknowledges that this Agreement represents the complete understanding and agreement between the Parties with respect to the subject matter contained herein. No other representations, stipulations, agreement, or understanding, whether oral or in writing shall be valid or enforceable or have any binding effect unless contained in this Agreement. Equipment Insurance. Insurance shall be maintained on all equipment and facilities, including fixtures, funded in whole or in part under this Agreement to replacement cost. The insurance shall include, at a minimum, insurance against loss or damage beyond the user's control, theft, fire or natural catastrophe. City shall be shown as lien holder on all policies.
Equipment Insurance. The Management Company shall cause to be carried and maintained insurance against all risks of physical loss or damage to the Equipment in an amount not less than the original purchase price or the replacement cost with like kind and quality at the time of loss, with such companies and as the Management Company shall reasonably determine.
