Equipment Allocation. One (1) unit of Equipment shall be furnished for every 25 State authorized Child Care attendees assigned to the Provider under the State Child Care Program (hereinafter “Active Participants”). CONDUENT reserves the right to remove excess Equipment on demand during Provider’s normal business hours. Excess Equipment is defined as a ratio of Equipment to Active Participants of less than 1:25 when more than 1 (one) unit of Equipment is furnished (examples: 1:18 or 1:21). Guidelines for Equipment allocation are established under a separate contract between CONDUENT and the State.

Equipment will be allocated at a ratio of 1 unit of Equipment to 40 State authorized Child Care attendees (hereinafter “Active Participants”), with the following two exceptions: (1) Class A, R and M Providers who provide services for 39 or less authorized Child Care attendees will receive, at a minimum, one unit of Equipment; (2) Providers will receive an additional unit of Equipment if there is a “remainder” after dividing the highest number of authorized Child Care attendees by 40. Examples: (1.) A Provider with 1-40 Active Participants would receive 1 unit of Equipment; (2.) A Provider with 41-80 Active Participants would receive 2 units of Equipment; (3.) A Provider with 85 Active Participants would receive 3 units. If a single Child Care Provider operates more than one facility, these guidelines apply to each of the facilities. Conduent reserves the right to remove Excess Equipment on demand during Provider’s normal business hours. Excess Equipment is defined as any equipment that exceeds the equipment allocation pursuant to the Guidelines for Equipment allocation described above and in the contract between Conduent and the State of Louisiana.

One (1) unit of Equipment shall be furnished for every 30 State authorized attendees assigned to the Provider under the EPS Program (hereinafter “Active Participants”) {Name} reserves the right to remove excess Equipment on demand during Provider’s normal business hours. Excess Equipment is defined as a ratio of Equipment to Active Participants of less than 1:30 when more than 1 (one) unit of Equipment is furnished (examples: 1:30 or 2:60). Guidelines for Equipment allocation are established under a separate contract between {Name} and the State. {Name} will furnish Equipment under this Agreement to providers who do not currently have Active Participants, provided an Equipment deposit is furnished by the Provider under Exhibit B.
Equipment Allocation. The equipment necessary for the development of the Research project will be stipulated through a Loan Agreement, based on this contract and its provisions, said equipment will be returned to “PPD” or “THE SPONSOR”, at the conclusion of the Study. Patient Travel Reimbursement: Patient travel costs not to exceed the rate set forth in the budget per randomized patient per Protocol defined scheduled visit will be paid to “THE INSTITUTE” for patient reimbursement upon receipt of invoice and supporting documentation. Any patient travel cost reimbursement exceeding this amount must have received prior written approval from “SPONSOR” in order to be eligible for payment. Serious Adverse Events (SAEs): SAEs will be reimbursed in accordance with the rate set forth in the budget for each SAE including the initial report and any follow up reports upon “THE SPONSOR” receipt of correct and itemized invoices from “THE INSTITUTE”. This reimbursement covers the time associated with the review and submission of the SAE.

Sponsor, at its reasonable discretion, through CRO, may provide equipment or reimburse reasonable cost to Institution for procuring/leasing/renting equipment needed for the Study. The Institution shall have responsibility to maintain and calibrate the equipment during Study and after the Study is completed. At the end of the Study, or in the event of early termination of the Study at Institution, Institution must return all leased/rented equipments and for purchased equipments where permitted by applicable law and regulation the equipment may become the property of the Institution and Sponsor shall have the right to decide whether the equipment is returned to Sponsor or not based on the amortized value. Zdravotnické zařízení bude odpovědný za to, že zařízení bude v průběhu klinického hodnocení i po jeho skončení udržováno a kalibrováno. Na konci klinického hodnocení nebo xxx xxxx xxxxxxxxxx xxxxxxxx u Zdravotnického zařízení je povinno všechno najaté či vypůjčené zařízení vrátit; pokud jde o zakoupené zařízení, pak pokud to platné zákony a předpisy dovolují, se zařízení může stát majetkem Zdravotnického zařízení a zadavatel bude oprávněn rozhodnout (s ohledem na amortizovanou hodnotu), zda se zařízení zadavateli vrátí nebo nikoli. CRO shall reimburse one off non-refundable start-up fee for preparation of the clinical study in the amount of 25.000 CZK based on the invoice issued by Medical Facility after the execution of this Agreement. CRO shall reimburse one off non-refundable archiving fee for archiving of study documentation in accordance with applicable law for duration of 15 years at the Medical Facility in the amount of 75.000 CZK based on the invoice issued by Medical Facility after the end of the Clinical Study. CRO uhradí jednorázový nevratný start-up poplatek za přípravu klinického hodnocení ve výši 25.000,- Kč, a to na základě faktury vystavené Poskytovatelem po podpisu této smlouvy. CRO uhradí jednorázový nevratný archivační poplatek za archivaci studijní dokumentace v souladu s příslušnými právními předpisy po dobu 15 let u Poskytovatele ve výši 75.000,- Kč, a to na základě faktury vystavené Poskytovatelem po ukončení klinického hodnocení.
