Equipment Allocation. One (1) unit of Equipment shall be furnished for every 25 State authorized Child Care attendees assigned to the Provider under the State Child Care Program (hereinafter “Active Participants”). CONDUENT reserves the right to remove excess Equipment on demand during Provider’s normal business hours. Excess Equipment is defined as a ratio of Equipment to Active Participants of less than 1:25 when more than 1 (one) unit of Equipment is furnished (examples: 1:18 or 1:21). Guidelines for Equipment allocation are established under a separate contract between CONDUENT and the State.

Examples: (1.) A Provider with 1-40 Active Participants would receive 1 unit of Equipment; (2.) A Provider with 41-80 Active Participants would receive 2 units of Equipment; (3.) A Provider with 85 Active Participants would receive 3 units. If a single Child Care Provider operates more than one facility, these guidelines apply to each of the facilities. Conduent reserves the right to remove Excess Equipment on demand during Provider’s normal business hours. Excess Equipment is defined as any equipment that exceeds the equipment allocation pursuant to the Guidelines for Equipment allocation described above and in the contract between Conduent and the State of Louisiana.

This reimbursement covers the time associated with the review and submission of the SAE.
Equipment Allocation. Sponsor, at its reasonable discretion, through CRO, may provide equipment or reimburse reasonable cost to Institution for procuring/leasing/renting equipment needed for the Study. The Institution shall have responsibility to maintain and calibrate the equipment during Study and after the Study is completed. CRO shall reimburse one off non-refundable start-up fee for preparation of the clinical study in the amount of 25.000 CZK based on the invoice issued by Medical Facility after the execution of this Agreement. CRO shall reimburse one off non-refundable archiving fee for archiving of study documentation in accordance with applicable law for duration of 15 years at the Medical Facility in the amount of 75.000 CZK based on the invoice issued by Medical Facility after the end of the Clinical Study.
