Common use of Employee Contribution Clause in Contracts

Employee Contribution. The employee shall pay all costs for medical, dental and vision benefits (based on illustrative rates determined by the third-party administrator unless otherwise agreed to by the parties in writing) in excess of the applicable Employer contribution set forth above. This employee contribution shall be split into 24 equal deductions and deducted from the employee’s paycheck on the first and second paycheck of every month.
