Drug Card. Employees covered under the drug plan will receive a drug card where the drug cost will be directly billed to the insurance company.
Drug Card. The Corporation of the Township of Ear Falls understands the financial burden on families paying out the cost of benefits, even though they are reimbursed by the Benefit Plan, the duration can be lengthy. To correct this issue, The Township of Ear Falls will provide a Benefit Drug Card to reduce this out of pocket expense.
Drug Card. A Drug Card will be provided to members who are covered by the Extended Health and Dental Plan and it will include: • dispensing of generic brand drugs in all cases except where the physician has indicated that no substitute brand is permitted and • a dispensing fee maximum of $8.00 per prescription.
Drug Card. The company agrees to provide regular employees a drug card, subject to automatic co-pay, a $2.00 prescription deductible and a generic drug program. The current deductible will remain in place.
Drug Card. There shall be a Drug Card with a $7.50 dispensing fee cap and a $1.00 deductible per prescription. A Positive Enrolment provision will be included. Reimbursement for prescribed drugs covered by the Plan will be based on the cost of the lowest cost interchangeable drug, unless there is a documented adverse reaction to the drug or where the employee’s doctor stipulates in writing that there are other medical reasons why the lowest cost interchangeable drug cannot be prescribed.
Drug Card. A personalized drug card from the Human Resources Department should be used to purchase prescribed drugs. The card may be used by you, or any eligible member of your family and is accepted by your pharmacist as payment.
Drug Card. Article 22.01 shall be amended to provide employees a Drug Card for prescription drugs. Any proposals that have not been addressed above have been considered and rejected by this Board of Arbitration. Accordingly, we Order the parties to incorporate the terms set out above, together with their agreed-upon items, into their renewal agreement as soon as practicable. Drug Card. For the length of this contract: PPO, HMOI and HMO Blue Advantage Drug Co Pays $15/$30/$60 (retail) $30/$60/$120 (mail) 7. Co-Pays – PPO , HMOI and HMO Blue Advantage $20 co-pay for Doctor’s Office visits and a $200 co-pay for Emergency Room visits. This $200 co-pay is waived if admitted to the hospital.
