Domestic Frame Relay Service. In lieu of any other rates and discounts, the Customer will pay fixed monthly recurring port charges (CIR) charges ranging from $80 to $2,005 for domestic Frame Relay Service based on port speeds ranging from: 56/64 kbps to 44.18M. In lieu of any other rates and discounts, the Customer will pay fixed monthly recurring PVC charges (CIR based) ranging from $3.00 to $5,767.75 for domestic Frame Relay Service based on PVC speeds ranging from: 16 kbps to 43.008M. Usage based PVC will range from $0.80 to $2,669.84 per minute. Ethernet Access: In lieu of any other rates and discounts, the Customer will be pay a fixed monthly recurring charge of $6,899.60 for Ethernet Virtual Private Line Service based on 600 Mbps speed. NRC is $0.00. A one year term commitment applies. If Customer terminates the circuit term before the end of the one year term, Company reserves the right to charge Customer for the remaining months in the circuit term. Discounts:

Domestic Frame Relay Service. In order for Customer to receive the special pricing in SCA Type 001 Option 3638 for Domestic Frame Relay service, no more than forty-five percent (45%) of Customer’s ports speeds may be 128k. If Customer is out of compliance with this condition, Company reserves the right to charge Customer an additional $150.00 per overage site per month and to modify the price for Domestic Frame Relay service going forward.   • Community Based Adult Intensive Service (AIS) and Child and Family Intensive Treatment (CFIT) – AIS/CFIT programs offer services primarily based in the home and community for qualifying adults and children with moderate- to-severe mental health conditions. These programs consist at a minimum of ongoing emergency/crisis evaluations, psychiatric assessment, medication evaluation and management, case management, psychiatric nursing services, and individual, group, and family therapy. In a Provider’s Office/In Your Home This plan covers individual psychotherapy, group psychotherapy, and family therapy when rendered by: • Psychiatrists; • Licensed Clinical Psychologists; • Licensed Independent Clinical Social Workers; • Advance Practice Registered Nurses (Clinical Nurse Specialists/Nurse Practitioners- Behavioral Health); • Licensed Mental Health Counselors; and • Licensed Marriage and Family Therapists. Psychological Testing This plan covers psychological testing as a behavioral health benefit when rendered by: • neuropsychologists; • psychologists; or • pediatric neurodevelopmental specialists. This plan covers neuropsychological testing as described in the Tests, Labs and Imaging section.

  • Station Service Seller shall be responsible for arranging and obtaining, at its sole risk and expense, any station service required by the Facility that is not provided by the Facility itself.
