Documentation of Services. Services provided by the COM faculty physicians will be documented in a manner that complies with State of Florida and federal laws and regulations governing direct and / or indirect financial relationships between physicians and the entities to which they refer patients for health care goods and services. The physicians will appropriately document clinical services provided hereunder and complete such other documentation as is required of physicians in order to bill and collect for professional services, including but not limited to completion of patient master billing form and attestation statement that the submitted billing is accurate.
