Designation of an Authorized Representative. Members have the right to designate an authorized representative who, in addition to the Member receiving services, will receive Explanation of Benefits forms from the PPO. If a Member wishes to designate an authorized representative, they must complete and sign an Authorized Representative form. This form can be obtained by calling the Customer Service Team at the telephone number indicated on the back of the Member Identification Card.

Designation of an Authorized Representative. An “authorized representative” of a claimant may act on behalf of the claimant in pursuing a benefit claim or appeal of an Adverse Benefit Determination, as required by 29 C.F.R. § 2560.503-1(b)(4), provided that the Plan Administrator may establish reasonable procedures for determining whether an individual has been so authorized to act on behalf of the claimant, and provided further that, in the case of a Claim Involving Urgent Care, a Health Care Professional with knowledge of the claimant’s medical condition shall be permitted to act as the claimant’s authorized representative with respect to that claim. To the extent required by law (including the Affordable Care Act guidance), references to “claimant” in this Article XI include a claimant’s authorized representative.
