Whereas, the City has established and currently sponsors a self-insured Employee Welfare Benefit Plan, to provide certain benefits (attached hereto as Exhibit "4" and hereinafter called the "Group Health Plan or "GHP") for covered group members and their covered dependents; and

The Work is the collection of solid waste, recyclables, and yard waste from all single family detached and attached homes in the City, as more fully described in Articles III through VI of the Form of Agreement included in this Request for Proposals. The specific service alternates and optional bid items to the Work are described below.
DESCRIPTION OF SERVICES TO BE PROVIDED. The City of Aledo invites sealed Xxxxxx’s Proposals for the Work generally described as the collection and disposal of Solid and Recyclable Waste from all Residential Dwellings in the City. Presently, residents choose their waste haulers individually. Approximately 1596 single-family, duplex, triplex, and multi-family households will be included in the curbside/parkway garbage pick-up. Bids should include curbside residential waste pick-up once per week and recycling pick- up.
DESCRIPTION OF SERVICES TO BE PROVIDED. Educational and Behavior Support Service consultations by a licensed and school-certified psychologist or by staff operating under the direct and ongoing supervision of a licensed and school-certified psychologist with individual students, The Charter School staff, BHR Service providers, and/or family members of students at times and places as agreed. Collection of behavioral information and related data from time to time as necessary to initiate and/or continue the delivery of appropriate services and to document same. It is understood that additional services (BHR or EPSDT services, outpatient psychological evaluation or treatment, etc.) may be provided to students of The Charter School by Consultant or employees of Consultant but will not be compensated by The Charter School except as approved by the Chief Operating Officer of The Charter School in writing in advance.
