Common use of Dental Insurance Plan Clause in Contracts

Dental Insurance Plan. The District will pay the premiums and provide to each eligible employee a family dental plan. This plan will provide the following defined benefits for the life of the Agreement and consist of the benefit levels as follows: 80 percent on Class I, Class II, Class III, and Class IV services. Class IV services have a dollar benefit lifetime limit of $800, and are only available for enrolled dependents under the age of 19. Contributions shall begin, in the case of new enrollees, at the beginning of the insurance month immediately following the time they begin their duties, provided, however, that employees submit the necessary application documents. Coverage will terminate at the expiration of the contract. Benefits are defined as follows:
