Dental Fund. 1. Full time employees. The Employer shall pay to the Local 338 Dental Fund for each of its full time employees covered by this Agreement the sum of $15.00 per month; effective April 1, 1984, $18.00 per month; and effective October 1, 1985, $20.00 per month.

Dental Fund. 28.01 The Corporation agrees to pay full premiums for all full-time employees to participate in the Corporation’s Dental Plan. Coverage will begin after the appropriate waiting period. Upon ratification, the Corporation also agrees to pay the amount of thirty cents ($0.30) for each hour paid to each part-time employee who has completed his or her probationary period. Effective September 1, 2007 the Corporation agrees to contribute the amount of thirty-five cents ($0.35) for each hour paid to each part-time employee who has completed his or her probationary period.

  • Dental specific medications for dental purposes, including fluoride medications (except for children less than five years of age with a non-fluorinated water supply);

Any premium paid by the State in excess of fifty thousand dollars ($50,000) coverage is subject to a tax liability in accord with Internal Revenue Service regulations. A supervisor may decline coverage in excess of fifty thousand dollars ($50,000) by filing a waiver in accord with Minnesota Management & Budget procedures. The basic life insurance policy will include an accelerated benefits agreement providing for payment of benefits prior to death if the insured has a terminal condition.
