Common use of Dental and Vision Clause in Contracts

Dental and Vision. The City shall reimburse each employee an amount 2 not to exceed 3 Two Hundred Fifty ($250.00) Dollars per single plan employees or Five Hundred ($500.00) 4 Dollars for two person or family plan employees for any dental and vision services 5 incurred by them or their family in the year for which reimbursement is sought. Such 6 reimbursement request shall be submitted directly to the Police Chief who shall then 7 submit the request to the Finance Director for reimbursement. 8 9 Employees will be eligible to enroll in the City dental coverage but must pay 100% of the 10 premium. 11
