Deliverables and Timeline. 1. Role of the Contractor The general scope of the contractor’s work will be to develop and conduct trainings for front-line care managers’/care coordinators as part of the State of Vermont's Integrated Communities Care Management Learning Collaborative. The primary goal of these trainings will be to strengthen the skills of front-line staff workers who provide care coordination services to high-need, complex patients in an effort to support better outcomes and reduce costs. The Contractor will develop and implement core competency training consisting of eleven training modules that cover disability-related topics from each of the folowing three categories: Category 1: Basic skills in care management/care coordination.
