Cost for Services. (a) In consideration of the Services provided by Holdings to Fantex as detailed in this Agreement, Fantex shall pay Holdings an amount equal to 5% of the gross cash received by Fantex pursuant to its brand contracts, if any, for any quarterly period (the “Service Fee”), as reported in its periodic reports pursuant to the Securities Exchange Act of 1934, as amended, for such period filed with the Securities and Exchange Commission (each, a “Periodic Report”). The parties may negotiate a change to the Service Fee in good faith from time to time to the extent that the parties believe that the Service Fee is not reasonably related to the actual cost of Services in any fiscal year.
Cost for Services. (a) In consideration of the Services provided by Safeway to Blackhawk as detailed in this Agreement, Blackhawk shall pay Safeway an amount equal to the actual amount of incremental cost to Safeway in providing the Services to Blackhawk which will be agreed to by the parties from time to time depending, in part, upon the Services requested by Blackhawk. The parties will negotiate and estimate all costs in good faith.

We will accept any applicable third-party reimbursement such as private insurance or public health insurance (Medicaid, Medicare). In addition, the Center receives funding from the State of New Jersey and Xxxxxx County which can be used to offset the cost for services to clients unable to pay the standard fee. Therefore, if your income qualifies you for our "ability-to-pay allowance", the portion of the fee for which you are responsible will be calculated upon your ability to pay. If you request an ability-to-pay allowance, proof of income is required at the time of intake (last month's pay stubs, last income tax return, entitlement program award letter). With the exception of Medicaid recipients, all clients are expected to pay their portion of the full fee. Clients with Medicare or insurance are expected to pay any applicable deductibles and/or their office co-pay at the time of each visit. Mille Lacs County will pay the School District/the Milaca Community Education Program $499.00 per month for the costs and expenses associated with providing adult education services to the Mille Lacs County Jail. Such amount shall be due and payable on the first day of each month for a period of three years beginning July 1, 2013 through June 30, 2016.

Cost for Services. (a) In consideration of the Services provided by Royalty Exchange to Royalty Flow as detailed in this Agreement, Royalty Flow shall pay Royalty Exchange an amount equal to 5% of the gross cash received by Royalty Flow pursuant to its royalty portfolio, if any, (the “Service Fee”). The parties may negotiate a change to the Service Fee in good faith from time to time to the extent that the parties believe that the Service Fee is not reasonably related to the actual cost of Services in any fiscal period.
Cost for Services. The annual cost for service is two dollars and fifty cents ($2.50) per individual record (Fee based on one record per unit/home in community.). Total annual fee is: Total Records x $2.50 = $ . CLIENT will receive UNLIMITED utilization of the VOLO Village system. CLIENT will be billed, and will be responsible for, payment of annual charges at the beginning of each year of this agreement. Prices are for the lower 48 United States only. At the end of each year of service VOLO shall review CLIENT’s record count and invoices will reflect any increase based on the pricing above. CLIENT’s record count may deviate above the initial number listed by CLIENT by up to 10% during each year of the agreement without midyear adjustment or additional fees. The service will include up to five (5) pass codes for an authorized “Service” users. Database administration charges shall be waived for the initial CLIENT data load. After the initial CLIENT data load, any database processing performed by VOLO which is required to properly format CLIENT data will be subject to a database administration charge of $25 to $75 based on the size of the CLIENT data file.
Cost for Services. The total amount payable by DOM to the Consultant under this Contract shall be limited as described in the Consultant's Proposal and shall be at the rate not to exceed a total of , the total amount appropriated from the State General Fund to the DOM for this purpose, as supplemented by the allowable Federal Medical Assistance Percentage for administrative services for the term of the Contract. All payments and terms thereof shall be made as described in the RFP.
